Behavioral Health Care Coordinator

Full Time | Day Shift | Monday-Friday | 8am-5pm

Experience based pay rate up to $28/hour

Lutheran Social Services of Central Ohio is currently seeking a Behavioral Health Care Coordinator for LSS Health Center (formerly Health Center at Faith Mission) in Columbus, Ohio. What will I do as a Behavioral Health Care Coordinator with Lutheran Social Services?

  • Monitor psychiatric hospitalizations, inpatient admissions and discharges for all clients of the Health Center; Work with CCBHC partners and participate in care coordination agreements.
  • Serve as the point of contact, advocate, and information resource for clients, care team, family/caregiver(s), and community resources.
  • Facilitates payer eligibility and client assistance programs.
  • Work with clients to plan and monitor care:
    • Assess client's unmet health and social needs.
    • Develop care plans with health and wellness goals and objectives.
    • Monitor progress toward care plan goals and facilitate changes as needed.
    • Timely follow-up with clients, staff, and community resources to coordinate care.
  • Creates care plans with the behavioral health care team for people with severe mental illness, co-occurring disorders, chronic health conditions and other high-risk needs in coordination with the client, primary care provider, and family/caregiver(s).
  • Facilitate client access to appropriate behavioral health, medical and specialty providers, treatment, and services identified in the Care Plan and meet with providers as needed to coordinate care.
  • Increase utilization of preventive care and multiple services within the health center.
  • Increase continuity of care by managing relationships with specialty care providers, transitions-in-care, referrals and monitoring client services across all levels of care.
  • Utilize DRVS and Transition of Care reporting to coordinate care across various care settings; including but not limited to intensive outpatient, residential care, emergency rooms, etc.
  • Increase clients' ability for self-management and shared decision-making.
  • Reduce emergency room utilization and hospital admissions when possible and coordinate care across all behavioral health levels of care.
  • Increase comprehension through culturally and linguistically appropriate education.
  • Connect clients to relevant community resources, with the goal of enhancing client health and well-being, increasing client satisfaction, and reducing health care costs.
  • Facilitate and attend meetings between client, family/caregiver(s), care team, payers, and community resources, as needed.
  • Participate in QI/QA and program planning, activities, data collection, and monitoring.
  • Assist reception staff with scheduling tasks and completion of intake documentation, including PRAPARE forms.
  • Administers NOMs assessment, re-assessment, discharge, and assist with other CCBHC grant reporting.
  • Assist Director and other BH team members with identifying clients with high utilization of services across all care settings; clients with severe mental illness and/or co-occurring disorders, chronic illness, high risk or vulnerability, and other special health care needs.
  • Recommend Care Coordination services or flag in EHR for monitoring.
  • Attend relevant training courses/webinars and meetings as necessary.
  • Provide feedback for the improvement of the Care Coordination Program and Behavioral Health services.
  • Collaborate with other staff on the interdisciplinary team and throughout the Health Center.

Requirements for a Behavioral Health Care Coordinator with Lutheran Social Services:

  • Bachelor's degree in healthcare, social work, public health, health education or related healthcare field preferred; or equivalent experience with Community Health Worker training.
  • Experience in social services required.
  • Health education experience desirable.
  • Experience in clinical or community resource setting.
  • Care coordination and/or case management experience is desirable.
  • Evidence of essential leadership, communication, education, and counseling skills.
  • Electronic Health Record experience preferred.
  • Proficiency with basic software (i.e. Word, Excel, Access, Outlook, PowerPoint, Internet Explorer, etc.).
  • Knowledge of community services and resources.
  • Valid Ohio driver's license, access to reliable insured transportation and ability to meet LSS insurance standards required.
