Essential Functions:
- Perform all tasks as required in the written plan.
- Meet productivity requirements as established by the program director or designee.
- Provide client monitoring and linkage to appropriate services both within LSSI and other community agencies.
- Complete and submit all necessary reports, billing, and documentation accurately and on time as required by agency and funding sources.
- Communicate with supervisor, work group and collateral agencies regarding individual's and /or family's status.
- Transport clients as needed.
- Work in collaboration with the network management team and/or other intra-agency entities on programmatic, fiscal, community and organizational issues.
- Commit to and actively participate in continuous improvement process.
- Demonstrate sensitivity and responsiveness to cultural differences present in the agency's service population.
- Perform other related tasks as necessary or as assigned.
Position Qualifications:
- High school degree required.
- Two years college preferred.
- Age requirement may apply.
- Basic knowledge of child development and parenting or equivalent life experience may be required or preferred in such networks.
- Demonstrate ability to communicate both verbally and in writing in a clear, comprehensible manner.
- Valid driver's license and proof of automobile liability insurance may be required.
