LSSI is hiring a full time ACT Team Lead, Monday through Friday from 9:00am to 5:00pm. Assertive Community Treatment (ACT) team on the Northwest side of Chicago. ACT Services consist of integrated crisis, treatment and rehabilitative supports provided by an interdisciplinary team to individuals with serious and persistent mental illness or co-occurring mental health and substance use disorders. ACT Services are intended to promote symptom stability, management of co-morbid health conditions, and appropriate use of psychotropic medications, as well as to restore personal care, community living, work and social skills. ACT Services encompass counseling and therapy, medication management and monitoring, skill building, and crisis stabilization services. ACT Services focus on the restoration of functional skills (e.g., psychosocial, adaptive, self-care) to promote and maintain community living.
 
The Program Assistant supports the team by organizing, coordinating, and monitoring all non-clinical operations of ACT. This includes managing medical records, including schedule appointments, tracking the team’s compliance with documentation, consents and HIPPA; maintaining accounting and budget records for individual and program expenditures, including assisting clients with Medicaid redeterminations; and providing receptionist activities, including triaging calls and coordinating communication between the team and client.

Essential Functions: 
  • Provide clinical supervision, training, guidance, and support to all Assertive Community Treatment (ACT) Team staff, including facilitating the daily staff meetings.
  • Participate in supervisory duties, including on-boarding/off-boarding of all ACT Team staff.  
  • Provide and coordinate a continuum of client treatment services. 
  • Carry a caseload of clients providing supportive counseling and concrete services and collaborate with other internal and external programs and treatment providers in order to provide these services. 
  • Ensure all documentation is completed in a timely manner and meets Rule 140/132 and/or MCO requirements. This includes crisis plan assessments and intake documentation.  
  • Comply with documentation and productivity standards. 
  • Transportation of clients in personal vehicle and agency vehicle is required.  Services will be provided at multiple locations.
Position Qualifications: 
  • Master’s in social service or related field required. 
  • LSW/LPC required; LCSW/LCPC preferred
  • 1-3 years of clinical experience is preferred
  • 1-3 years of experience working with mental health and/or substance use population is preferred.
  • 1-3 years of increasing leadership responsibility.
  • Excellent written and verbal communication skills.
  • Experience leading groups is preferred.
  • Experience in crisis intervention and de-escalation is preferred
  • Proficiency in EHR documentation and review.
  • Familiarity with accreditation and licensure standards.
  • Excellent written and verbal communication skills.
  • Ability to work in a team environment.
  • Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office – Outlook, Word, Excel and PowerPoint) along with other related software.
  • Driving is required.
  • Access to reliable transportation is required.
  • Valid Driver’s License in good standing for the state of residency and insurance.
  • Valid Illinois statutory and liability insurance coverage is required.
