Lutheran Social Services of Illinois is a not-for-profit social service agency with more than 70 program sites throughout the state of Illinois. Our services include counseling, mental health, substance abuse treatment, housing, work with prisoners and their families and services for older adults and a variety of services for children including preschool, foster care, and adoption.
The full-time Care Coordinator Supervisor will be responsible for the daily oversight of the Care coordination team which includes high fidelity wrap around care coordinators and intensive care coordinators. Additionally, this position supports Care Coordinators in their day-to-day processes and job responsibilities by providing oversight of the delivery of care coordination services, training to assigned staff members and assists in care coordination services to children and families with behavioral health needs. The Supervisor will guide staff in the utilization of evidence-based practices (High Fidelity Wraparound) that are a team-based, strength-based care approach to service planning and coordinating care.
Schedule: Monday through Friday 9am to 5pm

- Provides individual and/or group supervision for care coordinators providing direct service to clients enrolled in the Pathways to Success program.
- Responsible for assigning clients to care coordinators and ensuring services are performed in an efficient, prompt, professional and accurate manner and provides guidance, when necessary.
- Monitors care coordinators case work, time frames and caseloads including all tasks required to ensure delivery of services.
- Conducts regular staff meetings to convey information and resources to promote the professional development of staff and establish a collaborative team environment.
- Works collaboratively with identified partners and stakeholders to develop a network of resources including other community agencies and organizations such as DCFS, schools, hospitals, and primary care providers.
- Participate in Child & Family Team Meetings.
- May accompany staff on home visits or completes home visits and care coordination assessments.
- Maintains certification in High Fidelity Wraparound Care Coordination, IM CANS, and other state required assessment tools and evidence based practices.
- Participate in the hiring of care coordinators and performance monitoring.
- Monitors staff productivity and signs off on staff completion of case files including IM CANS, treatment plan reviews, progress notes, and discharges as appropriate.
- Maintains up-to-date and accurate client files and databases, as required by organization/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory, licensing or accrediting bodies.
- Coordinates with other program Supervisors, Coordinators and Directors as needed.
- Participation in HFS Pathways to Success provider meetings.
- Performs other related tasks as needed.
- Master’s Degree in Counseling, Psychology, Clinical Social Work, Marriage and Family Therapy or related field require.
- Supervisory experience required (1-3 years).
- Certification in High Fidelity Wraparound and/or IM CANS preferred.
- Experience working with children and families with behavioral health needs preferred.
- Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office – Outlook, Word, Excel and PowerPoint) along with other related software.
- Valid Illinois Drivers’ License in good standing and a vehicle are required.
- Proof of valid auto insurance.
Salary : $55,000 - $60,000
