LSSI is hiring a full-time Director of Clinical Services (LPN), Monday through Friday from 8:00am to 4:00pm. Responsible for providing supervision of the Supportive Living Program including completing resident assessments and overseeing the staff on the Supportive Living floors.

- Perform initial, quarterly, and annual resident assessments.
- Ensure all service plans and other required assessments are completed accurately and timely.
- Ensure all standards regarding safety, licensing, regulatory, and agency requirements are followed.
- Maintain resident files in accordance with HFS requirements, completing resident care plans.
- Perform medication set up and dispensing, communicate with pharmacy regarding resident medication and concerns.
- Perform on-call services as scheduled and respond to emergencies promptly.
- Maintain appropriate staffing levels.
- Provide direct supervision, training, support, review, and evaluation for assigned staff.
- Demonstrate professional, positive behavior and carry out responsibilities with integrity treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest and fair manner.
- High school diploma or equivalent required.
- Must be at least 18 years of age or older.
- LPN licensed by the State of Illinois, required.
- Supportive Living experience preferred.
- Certified in emergency resuscitation required.
- Minimum of five (5) years’ experience in healthcare services to adults with disabilities or the elderly population in assisted living, inpatient hospital, long-term care setting, adult day care, or in a department approved health related filed.
- Minimum of two (2) years’ experience in a supervisory/managerial role required.
- Background check clearance and Health Care Worker background check clearance required.
- Completion of a two-step Tuberculin test required.
- Demonstrated ability to communicate in a clear, comprehensible manner, verbally and written.
- Excellent organization, presentation and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
- Valid driver’s license, in good standing for the state of residency required.
- Access to reliable transportation required.
- Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
