LSSI is hiring a part-time Licensed Practical Nurse to administer prescribed medications, provide routine care, observe patients’ health, assist the primary care physician, and communicating with patients and their families in a long-term care facility.

- Monday Required shift 7am to 7am
- Tuesday through Sunday, as needed.
- PRN as needed to fill holidays, LOA, vacation, weekdays, and weekends 7am to 7am shifts.

Essential Functions:
- Administer medications as prescribed.
- Assess, plan, and evaluate nursing care in the facilities.
- Identify changes in clients' conditions and take appropriate action.
- Plan and provide direct care that maximizes functional capacities.
- Complete documentation in accordance with regulations and facility practices.
- Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, and other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest, and fair manner.
Position Qualifications:
- High School Diploma
- Valid IL LPN license.
- BLS/AED CRP trained.
- At least 5 years’ experience in long term care nursing.
- At least 5 years’ experience with individuals with developmental disabilities.
- Ability to provide leadership and training to direct care personnel.
- Participate in ongoing learning opportunities to keep skills current.
- Valid driver’s license, in good standing for the state of residency required.
- Access to reliable transportation required.
- Valid IL statutory.
