WHO WE ARE LOOKING FOR

The position of ACE (Ryan White) Housing Case Coordinator is designed to give one-to-one instruction and guidance in daily living skills to HIV persons who are enrolled in the HOPWA Program. Services include assessment of each participants’ ADLs, development of an instructional plan to address deficiencies, assisting clients in finding decent/affordable housing and furnishings, and case planning.

This position works closely with other members of the ACE team in the coordination and facilitation of linking clients with additional community services.

ESSENTIAL DUTIES

  • Maintains a caseload of 40 clients
  • Interviews new clients to assess problems/skills deficits in areas such as: budgeting, food prep and storage, nutrition, clothing care, personal hygiene, housekeeping, planning and keeping of appts, weekly calendar and organization, medication compliance
  • Evaluate and address client and family needs on an ongoing basis
  • Develop an individual instruction plan for each client and provide written materials to assist client in goal setting and achievement
  • Monthly monitoring with each client to ensure progress toward service plan goals
  • Home visits and services
  • Maintaining information regarding medical, psychosocial, educational, substance use/abuse and other social/emotional factors in a confidential manner
  • Assist participants in finding decent, affordable housing and essential furnishings, utility coordination
  • Assist clients with moves and transitioning from one home to another
  • Check requests for payment of mortgage, rent, utilities
  • Counsel clients individually and in small groups regarding plans for meeting needs
  • Maintains current documentation of all instructional activities and meetings with clients
  • Prepare and submit reporting
  • Reassess clients on an ongoing basis to determine progress and appropriateness/effectiveness of program participation
  • Assist ACE MCM in development and implementation of individual service plans
  • Refer clients to Case Management for services as needed
  • Communicate and coordinate with other program staff to ensure that each client is adhering to program rules and treatment plans
  • Attend multidisciplinary staff meetings
  • Other duties as assigned

COMPETENCIES

Required:

  • Good clinical knowledge of human behavior. Good interpersonal skills.
  • Demonstrates positive attitude when dealing with patient/staff.
  • Must have excellent organizational skills. Ability to prioritize work and handle multiple tasks.
  • Working knowledge of computers/software such as MS Word, Excel, and Outlook.
  • Must be able to communicate orally and in writing. Maintains patient confidentiality.
  • Must be very detailed oriented and capable of thinking independently while exercising good judgment. Required to make independent judgments without supervision. Must be able to generalize, evaluations, or decisions based on sensory or judgmental criteria.
  • Must be able to perform under stress when confronted with emergency, critical, or unusual situations.
  • Must be capable of dealing with periodic cyclical workload pressures & levels of responsibility.
  • Requires the ability to work with people beyond giving and receiving instructions

EXPERIENCE & REQUIREMENTS

Required:

  • 4 year BS/BA degree in sociology or other related field required or 2 year AA degree or ASN/nursing degree
  • Possess and maintain a valid driver’s license, safe driving record, and auto insurance; must be able to transport clients to various locations by using own transportation
  • Screening required for this position: a Level 2 background check (company paid), a drug screen (company paid), and a driver's license check (company paid) - all at time of job offer

Preferred:

  • Associate’s degree preferred.
  • Minimum 1-2 years’ experience with HIV/AIDS or high-risk population preferred

Job Type: Full-time

Pay: $19.00 - $21.25 per hour
