SALARY : Dependent on experience and qualifications
RESPONSIBILITIES : The Client Navigator works closely with clients to help them develop personalized case plans, providing ongoing assessment and support to ensure their needs are met effectively. In addition, they handle all the necessary forms and reports to maintain accurate and thorough case documentation, which is essential for tracking progress and ensuring accountability. Building and maintaining strong working relationships with outside agencies and individuals who serve LSS clients is also a key part of the role, as it helps create a network of support and resources tailored to each client’s unique situation. Navigator will also facilitate family and individual wellness activities, and evidence-based wellness programming.
QUALIFICATIONS : A qualified applicant will have a Bachelor’s degree plus one year of experience working in a human service environment; or an equivalent combination of education and experience. The ideal candidate will be an effective communicator with proven skills in writing, listening, problem-solving, and interpersonal skills for relating to diverse individuals as a representative of the Agency. They are able to apply common-sense understanding to resolve problems and attend to details, and are able to deal with problems to work independently. This position requires some evenings and weekend work.
Must successfully complete all agency background checks. Requires valid driver’s license and ability to obtain approved driver status.
