Description
As an Unaccompanied Children (UC) Foster Care Case Manager, you will be responsible for providing casework and supportive services to children in foster care, which includes facilitating any family members identified or referred for potential sponsorship and assisting with sponsorship applications as needed.
In addition to casework and supportive services, you will also be responsible for communicating with stakeholders in an effective and timely manner to ensure case continuity. This position is expected to function effectively with moderate supervision while following the guidelines given on procedures, along with agency, state, and federal regulatory requirements.
Essential Functions
- Ensure that all assigned youth on your caseload and under your supervision receive mandatory services while in care, including but not limited to mental health, medical, recreational, educational, and other supportive services in accordance with program requirements.
- Develop and complete assessments, service and treatment plans, progress notes, incident reports, and any other necessary documentation in a timely manner.
- Create a trusting and safe relationship with the client and encourage personal growth.
- Maintain accurate and current information on foster children in care, ensuring compliance with documentation and reporting requirements.
- Actively participate in weekly staff meetings with the clinical team and stakeholders.
- Attend, facilitate, and assist with providing ongoing education and support services to foster parents.
- Attend weekly meetings with the supervisor to consult on cases, review case plans, and evaluate client participation and progress.
- Schedule meetings with foster parents and foster children on at least a monthly basis and address any concerns in service delivery.
- Translate both written and verbal communication for program clientele and staff, if bilingual.
- Maintain effective communication with community agencies or individuals involved in service provision.
- Attend internal and external training and apply newly gained knowledge in providing services.
- Attend and prepare for required court hearings (if applicable).
- Transport clients as needed.
- Explore potential community resources and refer Case Managers and families for support, as needed.
- Adhere to federal, state, program, and agency regulations.
- Work flexible hours and independently in the field.

Qualifications
Qualifications and Education Requirements
- Bachelor’s degree in Social Services or related field from an accredited college required.
- Master’s degree in Social Services or related field from an accredited college preferred.
- Bilingual English/Spanish required.
- Must have a clean and valid driver’s license.

- Strong judgment and sensitivity to various cultural backgrounds.
- Broad knowledge of social work principles, concepts, and methodology.
- Excellent verbal and written communication skills.
- Ability to work under stress or in a fast-paced environment.
- Ability to work independently and maintain confidentiality.
- Proficiency in computer and typing skills sufficient to perform essential job functions.
- Pass a criminal history screen, including state and local child protection agency registries.
- Participate in a system of 24-hour availability to clients and foster parents, as required.
