Cuban, Haitian Entrant Case Manager
Job Purpose:
- To provide comprehensive case management services to Cuban and Haitian entrants, ensuring their successful integration into the community. This includes assessing their needs, developing individualized service plans, coordinating access to resources, and providing ongoing support and advocacy.
Key Responsibilities:
- Conduct comprehensive assessments of clients' needs, including housing, employment, healthcare, education, and other social services.
- Develop individualized service plans based on clients' needs and goals, outlining clear steps towards self-sufficiency.
- Coordinate access to essential resources, such as housing assistance, job training programs, medical care, and legal aid.
- Provide ongoing case management support, monitoring clients' progress, and adjusting service plans as needed.
- Advocate for clients' rights and access to services, navigating complex systems and addressing barriers.
- Maintain accurate and up-to-date client records, documenting services provided and progress made.
- Collaborate with community partners and stakeholders to ensure a coordinated and effective service delivery system.
- Provide cultural orientation and support to clients, assisting them in adapting to a new environment.
- Conduct home visits and community outreach as needed to engage clients and provide support.
- Stay informed about relevant policies and regulations affecting Cuban Haitian entrants.
- Participate in regular team meetings and supervision sessions to discuss cases and share best practices.
- Prepare reports and presentations on program outcomes and client progress.
Qualifications
Required Education:
- Bachelor's degree in Social Work, Psychology, or a related field.
Required Experience:
- Minimum of two years of experience in case management, preferably working with refugee or immigrant populations.
- Experience working with Cuban and Haitian entrants.
- Demonstrated experience in providing culturally sensitive services.
- Experience in conducting assessments, developing service plans, and providing referrals.
- Experience in advocacy and navigating social service systems.
Required Skills and Abilities:
- Excellent communication skills, both written and oral.
- Bilingual proficiency in English and Haitian Creole highly preferred.
- Strong interpersonal skills and ability to build rapport with clients.
- Ability to work independently and as part of a team.
- Excellent organizational and time management skills.
- Proficiency in Microsoft Office Suite.
- Knowledge of relevant laws and regulations pertaining to refugees and immigrants.
- Ability to maintain client confidentiality and professional boundaries.
- Strong problem-solving and critical thinking skills.
- Ability to work effectively under pressure and manage multiple priorities.
- Cultural sensitivity and understanding of the unique needs of Cuban Haitian entrants.
