What are the responsibilities and job description for the Case Worker- West Seneca Regional Permanency Center position at Lutheran Social Services.?
Reports To: Program Manager
Position Status: Full-Time (40-hours per week)
Position Schedule: The standard schedule is 8 hours a day, Monday through Friday, with hours flexed between day and evening shift.
Salary Range: $19.50 to $28.89 per hour, commensurate with degree obtained and experience
Qualifications:
Bachelor’s Degree required. Master's Degree preferred. Any deviations from these requirements must have a waiver approved by the OCFS.
Must have own transportation, a valid driver’s license, and vehicle insurance as required by NYS law.
Bilingual in English and Spanish is preferred.
Job Duties:
- Participate in the pre-admission screening of all families involved in the OCFS Grant program.
- Participate in the assessment and team meetings for all clients.
- Participate as a member of the interdisciplinary team.
- Participate in the development and ongoing maintenance of the Service plans to build goals, tasks towards those goals, and action steps for the family.
- Assist the child, family/caregiver, and community/school staff with understanding the child’s needs and provide techniques and information to better respond to the needs
- Attend regularly scheduled meetings with the post adoptive and post-guardianship families to teach, motivate, supervise and reinforce the positive relationships in the home.
- Act as advocates and liaison for clients (youth and parents) with other professional service providers in the community.
- Facilitates strength-based, solution-focused and trauma-informed engagement, intake, assessment, referral/community linkage, interventions, and cross-agency collaborations
- Ensures accurate, thorough and timely completion of progress notes, safety assessments, pre/post assessment measures, service plans, correspondence and case related documentation.
- Provides individual and group education, training and support to improve parenting strategies, reduce risk, enhance family stability and promote children's safety & well-being.
- Supports high risk and disconnected youth and their families in stabilizing behaviors, enhancing supports and enhancing safety.
- Will provide support to other offices as needed
- Obtains clinical counseling and other related services for members of the family and assists parents/youth in identifying and resolving issues jeopardizing permanency.
- Preparation and completion of all reports, statistical information and other file requirements as per agency policy and state mandated regulations.
- Participates in the ongoing development of and preparation in training for treatment families.
- Empowers and encourages families to make use of all identified resources.
- Assists in marketing programs to referring agencies.
- Intervention in crisis or conflict situations in the treatment family homes assigned, moving towards the resolution of problems and redefining new goals.
- Participation in the on-call rotation.
- Participation in ongoing training as indicated by state regulations.
- Perform other tasks and duties as assigned by supervisor.
- As regulations and programmatic changes occur responsibilities may change to meet these needs and requirements.
- Safety – Follows required safety procedures.
- In-services – Attends all required in-service training.
GAFS Organizational Responsibilities:
- Actively participates as part of the interdisciplinary treatment team for assigned youth and to assist in coordinating and monitoring services that are tailored to the needs of the youth.
- Engages in regular supervision with own supervisor, including scheduled evaluations.
- Actively participates in all in-service programs and scheduled meetings
- Maintains ongoing and open communication with team members and promotes relationships of trust and respect with clients, families, customers, and staff.
- Responsible for emergency on-call coverage, as scheduled
- Fulfills obligations inherent in the major system of the GACFS and the LSS Personnel policies.
- Demonstrates a commitment to organizational and department goals.
- Shares positive and negative information directly with team members and confronts others; to utilize the organizational chain of command.
- Practices good listening skills and communicates well both in writing and verbally.
- Demonstrates personal and professional ethics, attitudes and value systems which enhance team functioning and to be abide by the Standards of Confidentiality.
- Upholds child and family rights as outlined in State Office of Child and Family Services mandates and GACFS procedures.
