JOB TITLE: CNA
DEPARTMENT: Home Care Agency
REPORTS TO: Home Care Director
SHIFT: 1st shift 7a - 3p, including working every other weekend
CLASSIFICATION: Non-Exempt
POSITION SUMMARY:
Responsible for assisting Home Care Agency clients with personal care and homemaker needs.
Essential Duties and Responsibilities:
- Use appropriate skills and abilities to support Home Care Agency clients with personal care needs.
- Review and follow current care plan developed for the client, and appropriately document care provided.
- Communicate with supervisor and Home Care Agency scheduler of availability to work specific days/hours, and provide timely requests for time off or other changes in availability.
- Attend departmental meetings and trainings, and comply with requirements for Relias Online Learning courses.
- Inform supervisor and nurses in the appropriate level of care of any changes or concerns with client functioning, health, and overall well-being.
Education and/or Experience:
- 2-3 years experience in healthcare field desired.
Certificates, licenses, registrations:
- Certified Nursing Assistant certification required.
