OVERVIEW :
The Admissions Coordinator is to assist in census development and support the organizations admission process. The Admissions Coordinator reports to and works in concert with the Center Director of Marketing and Admissions. The Admissions Coordinator will ensure that regulatory compliance is met by ensuring that activities are completed in accordance with the standard operating procedures as outlined by the organization.
RESPONSIBILITIES :
Reconcile Center Census daily
Maintain electronic bed board / room assignments and collaborate daily with Environmental team to ensure patient room readiness (acute DCs, bedholds, planned DCs, room moves, etc.)
Send Opening Census Report by 9 : 30AM to designated teams
Communicate all acute readmissions to Business Development team for follow-up, reach out to patient / family to ensure re-admit to the Center
Maintain and manage all department data logs and reports- tours, referrals, trend reports.
Assign and track facility Physician
Provide Closing Census Report by 5 : 30PM
Complete Admissions Checklist and Insurance Verification (MCR, Primary and Co-Insurance) processes
Check for outstanding balance on all Center readmissions, prior to accepting all referrals. Collect open balances prior to readmission into the Center.
Obtain necessary approvals on financial and clinical criteria for every all referrals.
Input all patient referral information into the necessary systems.
Communicate all questions and decisions to referral sources / Business Development team utilizing the appropriate systems
Collaborate with Business Development team to gather / collect necessary medical and financial documentation to present to internals teams for approval
Follow up daily with Business Development teams on the status of new and existing referrals to coordinate Center admissions
Distribute all necessary paperwork to pertinent staff prior to admission (nurses’ stations, social work, environmental) to ensure Center preparedness for patient arrival
Assist Director of Admissions and Marketing with facility event planning.
Attend UR meetings when Admissions Director if not present
Obtain DC forecast for the week, daily to be included in opening / closing census reports
Coordinate patient clinical needs on anticipated admission date from the hospital (special beds or equipment, medications, isolation, etc.)
Provide all anticipated planned patient admission information to MOD / Supervisor for follow up. Collect follow up report in morning meeting. Communicate necessary information to Business Development team
Complete the required admissions and billing tasks for each patient admission
Ensure that patient and family are appropriately welcomed and oriented to the Center on admission day
Complete admissions agreements for new / readmit patients within 24 hours of admission following Admission SOP
Collect appropriate insurance cards
Collect private pay funds where appropriate prior to admission (of PP resident)
Conduct tours for prospective patients, families or business affiliates and complete the necessary follow up
QUALIFICATIONS :
EDUCATION / EXPERIENCE :
2 years of experience in an Admissions / Marketing position in healthcare environment
Long term care experience required
Knowledge of Medicare and 3rd party billing
Knowledge of medical terminology
Ability to discuss clinical evaluations with physicians and other care givers within facility
SUPERVISORY RESPONSIBILITIES :
This position has no supervisory responsibilities
WORK ENVIRONMENT :
This job operates in a health care setting. This role requires regular walking to various locations around the care center.
Incumbent may be exposed to virus, disease and infection from patients and specimens in working environment.
Incumbent may be required to work extended periods of time at a video display terminal.
Incumbent may experience traumatic situations, including psychiatric, and deceased patients.
Works beyond normal working hours and on weekends and holidays when necessary
