Job Description:
As the Home Health Intake Coordinator/Scheduler, you will be responsible for managing the intake process, receiving and processing new orders from referral sources, ensuring all required documentation is complete, and coordinating home health services. Additionally, you will schedule home health visits and work closely with clinicians to ensure that services are delivered on time and in accordance with patient needs. You will communicate regularly with patients, caregivers, and healthcare professionals, and maintain accurate, up-to-date patient records while ensuring compliance with company policies and healthcare regulations.
Key Responsibilities:
The Home Health Intake Coordinator/Scheduler will manage the intake process by receiving and processing new patient orders from referral sources, ensuring that all necessary documentation is accurate and complete. In addition to intake, the position involves scheduling home health visits, coordinating with clinicians to ensure timely service delivery, and maintaining organized and up-to-date patient records. The role also requires effective communication with patients, caregivers, and healthcare professionals to ensure smooth coordination of services, all while adhering to healthcare regulations and company policies.
Qualifications:
- Previous experience as a Home Health Intake Coordinator/Scheduler is required.
- Strong communication and organizational skills.
- Ability to manage multiple tasks in a fast-paced environment.
- Proficiency with scheduling software and electronic health records (EHR) systems.
- Knowledge of home health regulations and compliance requirements is a plus.
