What are the responsibilities and job description for the Case Manager position at Lydia's House of Hope?

Lydia’s House of Hope is seeking a Part-Time Case Manager that is ready to multitask in a fast paced environment. We ask our Case Managers to be as good at working alone as they are working with a team, and be forward thinking, anticipating the needs of the residents that are in their care.If you are well rounded, with good boundaries and at least 3 years of experience working with a caseload of families that have come out of homelessness and/or poverty situations, you are just who we are looking for!

Treating each family with dignity and respect, the Case Manager interviews and conducts intake assessments with potential residents. They routinely meet with residents to discuss their progress, address any unmet needs, and connect them with necessary resources and/or social services. Regular follow up with residents is critical to this position and the health and well-being of the families. The goal is to provide the services and support required for the resident and their family to leave the program being empowered, independent, and self-sufficient.

This is a direct-care position that reports directly to the Executive Director.

Duties:

  • Facilitate 1:1 weekly face to face meetings with assigned residents to build relationships, discuss progress toward treatment plan goals, and address any other needs/concerns.
  • Complete Meeting Documentation During 1:1 weekly meetings that is used to create Weekly Reports to provide directly to the Executive Director
  • Demonstrate the ability to lead individuals through the understanding of generational poverty and ensures that the plan of self-sufficiency incorporates a comprehensive plan to those we serve.
  • Conducts an initial baseline assessment and continues with ongoing assessments to determine individual needs.
  • Develop a comprehensive 4-phase self-sufficiency plan based on the individual’s needs - addressing (but not limited to) parenting, health, social, financial, family, housing, educational, and job skills.
  • Assures access to services outlined in the plan through documented contact with referral sources, in house and outside services.
  • Works directly with service providers, coordinates services and avoids duplication of services.
  • Monitors and documents progress toward established goals.
  • Participate in ongoing consultation as needed with the Executive Director to discuss interventions, patterns of behavior and/or family needs and concerns.
  • Completes departure plan and determines the need for and referral to sustainable living.
  • Conduct thorough follow-up with each resident on a regular basis to ensure immediate needs are met, schedules are followed, programs are attended, and to determine if additional services are needed.
  • Oversee the dispensing of all medication and documentation of medication for each resident.
  • Create weekly schedules and oversee all daily living activities for each resident.
  • Provide safe transportation when necessary to meet the needs of all residents.
  • Be comfortable in executing all disciplinary actions as necessary
  • Develop positive and productive relationships with families served, community agencies, and law enforcement entities.
  • Maintain professional and ethical behavior toward families served, community agencies, and law enforcement entities.
  • Ability to adapt to sudden changes in daily plans.
  • Provide services in accordance with Lydia’s House of Hope policies and procedures and as assigned by the Executive Director.
  • Demonstrates knowledge of issues faced by those who experience poverty and homelessness.
  • Knowledgeable of local community outreach agencies and how to utilize them as needed in individual case plans.
  • Develop individual and comprehensive living and educational plans with persons served.

Qualifications:

  • Bachelor’s degree in Social Work or related field.
  • Knowledge of community resources and counseling/social work practices with high risk populations.
  • Minimum 3 years’ experience working with persons in crisis
  • Good documentation skills.
  • Excellent written and verbal communication, ability to establish rapport.
  • Ability to motivate others towards achieving goals.
  • Ability to work independently with strong sense of focus, task-oriented, nonjudgmental, open personal qualities, clear sense of boundaries.
  • A strong sense of and respect for confidentiality involving both clients and fellow employees.
  • Must be willing to undergo a background check in accordance to state and federal laws

Job Type: Part-time

Expected hours: No more than 25 per week

Schedule:

  • Day shift
  • Monday to Friday
  • No weekends
