As a Transportation Driver, you'll be responsible for safely transporting residents to medical appointments and activities, working collaboratively with our interdisciplinary team to ensure seamless care transitions.
Key Responsibilities:
- Provide safe and efficient transportation for residents in our center van or with a third-party provider.
- Accompany residents to medical appointments and activities, ensuring their comfort and well-being.
- Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each resident.
Qualifications:
- Certified Nursing Assistant (CNA) certification in good standing with the state or must complete state-required training and a competency evaluation program within four months of employment.
- A clean driving record and valid driver's license.
- Ability to relate positively to residents and families and work cooperatively with other employees.
- Meet all local health regulations and successfully complete a post-offer health assessment.
