Shifts:
Monday–Friday, Day Shifts
Key Responsibilities:
Assist the Social Services Director in assessing the social and emotional needs of residents.
Support the development and implementation of individualized care plans.
Help coordinate services to ensure residents' needs are met, including discharge planning and community resources.
Build and maintain relationships with residents, families, and external agencies.
Document social service interactions and maintain accurate records.
Provide emotional support and counseling to residents and families during difficult transitions.
Assist with the coordination of activities to enhance resident quality of life.
Address and resolve concerns raised by residents or their families in a timely manner.
Qualifications:
High school diploma or equivalent; an Associate’s or Bachelor’s degree in Social Work or a related field preferred.
Experience in a healthcare or long-term care setting preferred.
Strong organizational and communication skills.
Compassionate and patient-centered approach to care.
Ability to handle sensitive information with discretion and maintain confidentiality.
Knowledge of community resources and social service programs.
