Job Summary
The Director of Nursing acts as a clinical leader by overseeing of all aspects of clinical care within the nursing scope of practice throughout the clinic. The Director of Nursing is a key member of the Leadership Team, contributing to strategic vision of the organization, directly supervising the nursing team, and providing coverage for nursing support to LMCHS patients using a trauma-informed, harm reduction focused, patient-centered medical home delivery model. They participate in the Operations Committee, which oversees training and workforce development programs, new staff on-boarding, Quality Improvement activities, Research programs, and the development, implementation and maintenance of policies and procedures. The Director of Nursing is expected to uphold and imbue in others a commitment to addressing racism, ableism, fatphobia, gender and sexual discrimination within the organization and our clinical practice.
Key Responsibilities
Supervisor Responsibilities:
- Provides direct supervision of all Registered Nurses, the Outreach RN, and the Medical Assistant Manager, leading hiring, on-boarding, training, professional development, performance evaluations, and disciplinary actions for all direct reports. Manages schedules and coverage for the Nursing department, stepping in to provide direct coverage when needed.
- Develops, implements, and maintains policies & procedures, standing orders, guides, handouts, and forms pertaining to the Nursing Departments in collaboration with Operations Committee.
- Provides clinical leadership and oversight to students participating in Lyon-Martin’s workforce development programs such as its clinical rotation, externships, and fellowship program.
- Participates and/or leads research programs as needed.
- Serves on the Quality Improvement Committee, leading PDSA interventions and acting as a champion of change.
- Supports team in meeting benchmarks for applicable quality indicators as noted in the annual quality improvement plan.
- Leads and oversees quarterly nursing peer review processes.
- Increases staff competencies and satisfaction through training and mentorship.
- Works to improve the overall quality and efficiency of clinical workflows.
Clinical Care Responsibilities:
- Promotes and enhances integrated team-based care in the provision of medical care, including patient centered care using trauma-informed care and harm reduction frameworks.
- Provides care, within the scope of credentialed privileges and standardized procedures, to patients within an integrated delivery model, documenting care promptly and following charting protocols and applicable CA state licensing requirements; completes patient sessions in a timely manner; closes visit notes in a timely manner (same day for visit notes and urgent inbasket messages, within 72 hours for routine messages)
- Informs clients of normal and abnormal lab results and follow up plans, as appropriate for scope or under guidance of a provider.
- Triages and responds to patient and colleague requests for information and assistance (e.g., form completion, prescription refills, etc.) in a timely manner.
- Works collaboratively with mental health staff to facilitate integration between primary care and mental health in order to improve patient outcomes.
- Obtains and maintains licensure and BCLS/CPR certification.
- Attends continuing professional education programs, at the request or with the approval of supervisor, demonstrating participation in required hours of continuing medical education according to California law.
- Utilizes motivational interviewing skills and encourage patient activation and self-management.
- Appropriately utilizes standing orders.
- Facilitates team functioning and efficiency.
- Assesses, triages, and provides nursing advice to patients who walk-in with medical complaints or who call-in with medical concerns seeking advice.
- Prioritizes and takes responsibility for patients with complex medical issues and follow up with all patients regarding recent emergency room visits and inpatient stays.
- Works in collaboration with Leadership Team to review and makes recommendations for changes to all forms and handouts stocked to ensure evidence-based, trans inclusive and patient-centered language is used.
- In collaboration with Leadership Team, creates and provides trainings and orientation to RNs about LMCHS specific policies, procedures and protocols to newly on-boarded clinical staff.

Education, Knowledge, Skills:
- Graduation from an accredited school of nursing with minimum BS degree
- Possession of a valid, active California license as a Registered Nurse
- Possession of BCLS/CPR certification
- At minimum, 2 years experience in clinic setting, prefer 5 years
- At minimum, 1 year experience in supervisory role, prefer 3 years
- At minimum, 2 years experience working with Lyon-Martin’s priority populations in a healthcare setting
- Commitment to the mission of Lyon-Martin and to community-based primary care
- Knowledge of third party payer requirements, California practice guidelines and malpractice laws
- Demonstrated leadership and expertise in the field of transgender, queer, and intersex health
- Experience working with lesbian, bisexual, transgender, gender non-conforming, non-binary, and intersex clients, populations with varying lifestyles, disabilities, racial, ethnic and cultural backgrounds and economic statuses, clients who do sex work, those experiencing homelessness, people who have challenges with mental illness, people who use drugs, those of with a history of incarceration, people of varying ages, safety net populations.
- Commitment to working in an environment offering abortion services and the full scope of pregnancy options counseling.
- Experience with electronic medical records, specifically Epic.
- Strong phlebotomy skills.
