Pay:    $106.14 per hour (MD or DO)

            $72.48 per hour (NP or PA)

The mission of Lyon-Martin Health Services is to provide high quality, compassionate and trauma-informed medical, gynecological, and mental health care services to trans, non-binary, gender non-conforming, and intersex communities and cis-gender women with specific sensitivity to LGBQA sexual orientation, disability, size, race, ethnicity, and language, regardless of immigration status or ability to pay.

Job Summary

It is the responsibility of the Per Diem Primary Care Provider (PDPCP) to act as a clinical leader by providing quality primary care services to Lyon-Martin patients, helping to grow the overall practice and promote team functioning.  The PDPCP acts collaboratively with a supervising physician according to California state law.  They are also responsible to uphold and imbue in others a commitment to addressing racism, ableism, fatphobia, and gender and sexual discrimination within the organization and our clinical practice.

Under the supervision of the Medical Director, the PDPCP is responsible for jumping in where needed to fill any coverage gaps of staff medical providers. When on duty, PDPCP responsibilities include taking medical histories, performing physical examinations, ordering and interpreting laboratory tests, diagnosing and treating illnesses and injuries, and referring out when necessary.  The PDPCP provides consultation and education to patients using a trauma-informed, harm reduction framework, including but not limited to chronic disease management, pain management, substance use disorder treatment, gender affirming care and gynecological care. The PDPCP cares for the medical needs of patients presenting with co-occurring disorders and barriers to care within the Patient Centered Health Home delivery model. The PDPCP is an active member of the integrated healthcare team, working collaboratively with Mental Health Providers and Specialists. May be required to work evenings and weekends.

Key Responsibilities

  • Provide comprehensive care, within the scope of credentialed privileges, to Lyon-Martin clients
  • Document care promptly, closing visit notes within 72 hours, responding to in basket messages with 72 hours, following charting protocols and applicable CA licensing requirements.
  • Analyze and interpret patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.
  • Ensure appropriate follow-ups for all aspects of ordered care (e.g., specialty referrals, laboratory results, radiology reports, etc.).
  • Consult with the Medical Director, Associate Medical Director and/or other consulting physicians for treatment options as needed.
  • Maintain familiarity with the standardized procedures as agreed to by the supervising physician and utilize them to guide care.
  • Complete patient sessions in a timely manner.
  • Provide clinical leadership to support staff, including medical assistants, patient care coordinators, front office staff and receptionists.
  • Work collaboratively with mental health staff and specialists to facilitate integration between departments to improve patient outcomes.
  • Meet benchmarks for quality indicators as noted in the annual quality improvement plan.
  • Along with colleagues, assist in continuous quality improvement through on-going needs assessment and problem solving.
  • Maintain availability for various administrative/clinical duties.  Such duties may include case presentations; in-service training to staff; supervision of students, interns, residents, nurses, and medical assistants; leadership/consultation to agency committees; attendance/speaking at fundraising events, etc.
  • Attend continuing professional education programs, at the request or with the approval of supervisor, demonstrating participation required professional education to maintain certification/licensing.
  • Represent Lyon-Martin at and/or attend professional conferences, in-service trainings, and meetings.
  • Obtain and maintain licensure, DEA registration and BCLS/CPR certification.
  • Complete all mandatory trainings on an annual basis.
  • Comply with all aspects of Lyon-Martin Policies and Procedures applicable to Medical Staff members as well as all agreements with insurers and other entities that contain provisions applicable to or affecting the duties and responsibilities of Medical Staff; and all applicable statutes, regulations, governmental agency guidelines, and non-governmental oversight body requirements that place requirements on Lyon-Martin.

Education, Knowledge, Skills: 

  • At least 1 year experience as a licensed, practicing clinician.
  • Graduation from an accredited program for Nurse Practitioners or Physician Assistants, Doctor of Medicine (MD) or Doctor of Osteopathic Medicine.
  • Possession of a valid California license as an AGNP or FNP with Furnishing license OR Possession of a current, valid California License to practice as a Physician Assistant issued by the California Board of Medical Quality Assurance OR valid California License to practice as a Physician and Surgeon or Osteopathic Physician and Surgeon.
  • Possession of DEA Controlled Substances Registration Certificate
  • Possession of NPI 
  • Current BLS/ACLS certificate
  • Experience working with lesbian, bisexual, transgender, gender non-conforming, non-binary, and intersex communities.
  • Experience working with populations with varying disabilities, racial, ethnic and cultural backgrounds and economic statuses.
  • Experience working with people who do sex work, those experiencing homelessness, people who have challenges with mentally illness and/or using drugs, those of with a history of incarceration, people of varying ages.
  • An interest in and commitment to gender affirming health care and health care access for underserved populations.
