Lyons Primary Care Group in Greystone, AL is seeking a dedicated and compassionate LPN to join our team. The ideal candidate will play a vital role in delivering high-quality, patient-centered care in a primary care setting. This position involves working collaboratively with other healthcare professionals to ensure the optimal health and well-being of our patients.

Qualifications

Active LPN license in Alabama.
Associate Degree in Nursing (ADN)
Minimum of 1-2 years of nursing experience, preferably in a primary care, outpatient, hospital, family medicine, or similar setting.
Strong communication and organizational skills.
Ability to multitask and maintain attention to detail in a fast-paced environment.
Proficient in electronic medical record (EMR) systems.
Strong phlebotomy and injection skills.
Successful completion of nursing training and certification as a nurse from an approved program.

Job Summary & Responsibilities

Provide direct care to patients, including assessments, treatments, and follow-up care, based on individualized plans.

Drawing blood / venipunture, obtaining specimen for lab work

Medication injections

Document patient history, symptoms, and health insights accurately in medical records.

Providing scribing and data entry for providers as needed.

Administer medications and vaccinations safely, in adherence to physician orders and best practices.

Assist with minor procedures, wound care, and routine medical interventions as required.

Educate and guide patients on managing chronic illnesses, preventive care, and healthy lifestyle practices.

Coordinate with multidisciplinary teams to ensure continuity and quality of care.

Monitor patient progress and proactively address any health concerns or complications.

Maintain compliance with all regulatory health and safety guidelines.
