Position :
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. The ideal candidate will have a strong commitment to patient care and the ability to work collaboratively with a multidisciplinary team. You will play a vital role in supporting our residents’ medical needs and enhancing their quality of life.
Key Responsibilities:
- Provide direct nursing care to residents, including administering medications/injections, providing wound care, monitoring vital signs, and assisting with daily living activities.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Support residents in their rehabilitation goals and promote independence through personal care and therapeutic activities.
- Maintain accurate medical records and documentation in accordance with regulations and standards.
- Educate residents and staff on health management and care techniques.
- Ensure a safe and clean living environment for all residents.
- Participate in team meetings and contribute to the overall improvement of care practices.
- Oversees MAP Program at the residence by conducting routine audits of documentation system, providing specific and systemic response to medication errors.
- Conducts reviews of health care documentation to ensure program compliance with the provider’s recommendations as well as MAB, DDS policies and the MAP policies.
-Develops and conducts in-service training to Adult Disability Service employees.
-Identifies and facilitates the utilization of appropriate community resources.
-Provides client-specific consultation to program staff, in conjunction with health care providers.
-Transports participants to any medical appointments and provides advocacy at the appointment. Completes all follow up from the appointment.
Qualifications:
- Current state licensure as a Licensed Practical Nurse (LPN).
- Previous experience in a healthcare setting, preferably with individuals with brain injuries or other neurological conditions.
- Strong communication and interpersonal skills.
- Ability to work effectively as part of a team and independently.
