Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty and addictions. Located in the heart of the Old Town Chinatown neighborhood, Old Town Clinic supports compassionate providers and treatment for acute and chronic health concerns, primary care, mental health support, care for substance use disorders and an on-site pharmacy.
The Health Assistant (HA) works as a member of a multi-disciplinary primary care team providing integrated primary care services to homeless and low-income patients seen at Old Town Clinic. The Care Team is responsible for proactively managing the healthcare needs of the patients on their assigned panel, including behavioral and medical health needs. The HA is responsible for providing support to the Care Team Managers (CTM) and providers on each primary care team.
Schedule : Monday - Friday, 8 : 00am to 5 : 00pm
Location : Old Town Clinic (727 W Burnside Street. Portland, OR 97209)
Compensation : $23.14 per hour

MINIMUM QUALIFICATIONS
- High school diploma or GED required. Prefer Bachelor’s Degree or higher in health or social.
- Must have current CPR certification before start date.
- At least one year experience working with homeless, low-income, or other vulnerable populations in a supportive role preferred.
- At least one year experience working in a clinic setting preferred.
- Spanish language fluency preferred.
- Has the competencies necessary to : Communicate effectively orally and in writing; Understand healthcare related terminology; Provide outreach, engagement and supportive services
- Ability to adhere to Central City Concern’s drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Work Place Ace of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol.
- Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS background check unit.
- Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects.
- Must adhere to agency’s non-discrimination policies.
- Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, political affiliation, cultural backgrounds, life-styles and sexual orientations, and treat individuals with respect and dignity.
