The Hospice Program Coordinator provides administrative support forthe delivery of Hospice and Palliative services for patients, families, and volunteers. This role works closely with the HospiceDirector in the day-to-day execution of non-clinical hospice services includingpatient record management, referral, intake, scheduling, and volunteer programsupport. Additionally, the ProgramCoordinator completes reports and maintains program compliance according to NHPCOand WSHPCO (state hospice organization) standards plus Medicare conditions ofparticipation. TheHospice Program Coordinator is often the first point of contact with families andthus serves as a brand ambassador for Wesley. The coordinator should be well-versed in the hospice philosophy andshould be comfortable speaking to families regarding Hospice and PalliativeCare Services.
Salary Range : $21.60 to $31.77 per hour
