- Day Habilitation Provider is a provider specializing in the skill of assisting individuals to maintain, learn, or improve skills and functioning for daily living. He/she must have the ability to communicate both orally and in written form, with all members of a team. Dependability and punctuality is a priority, and a caring personality with a heart for individuals with disabilities is a must. Must have the ability to handle physical aspects of job, including: bending, stooping, lifting, pushing, pulling, reaching, and walking for periods of time. Hours for this position can vary depending on the individuals being served.
Day Habilitation Provider pay ranges from $18.00-$20.00 an hour.
General responsibilities include:
- Provide reliable transportation to support the individual/s being served.
- Provide professional day habilitation, supported living/in home support, and hourly respite services, as authorized, to individuals served. Person centered services is of the up most importance. This position may include: Teaching clients self-care skills, assisting clients with daily tasks, helping clients to be successful with the goals they have set, and being safe and healthy in their community.
- Assist with creation and development of goals and objectives, along with the proceeding methodology
- Provide quality data to be used for quarterly summaries. This is done with detailed, accurate, and timely documentation.
- Maintaining safe work environment at all times.
- Plays active role in the individual’s team including assisting with the creation, teaching, and utilizing positive behavior support plans.
- Aid with budgeting, banking, and receipt collection.
Minimum Requirements:
- A minimum of 1 year experience providing direct care services, however, we are willing to provide training to the right candidate
- At least 18 years old
- High school diploma or General Equivalency Diploma
- Possess a valid Alaska driver’s license, a good driving record, and have insurance on the vehicle used for work.
- Can provide a current certified CPR/First Aid
- Has Current TB Test
- Attend all assigned/scheduled trainings
- Must be able to pass a fingerprint background check
- Must be able to provide 2 professional references and 3 personal references.
Job Type: Part-time
Pay: $18.00 - $25.00 per hour
