JOB SUMMARY
Coordinates the reception area activities to promote communication throughout the Facility. Performs clerical and reception duties. Maintains the Facility’s medical records according to established guidelines and requirements. Reviews records for compliance with approved policies. Assures all operative reports completed.

QUALIFICATIONS
Good communication skills required
Two years clerical experience preferred
Medical Clerical experience preferred
Requires language skills adequate for written and interpersonal communication in American English
Requires visual and auditory acuity adequate to monitor patient and visitor activity and for frequent use of computers and occasional use of other business office equipment
Ability to sit for long periods and to perform desk and office activities
OCCUPATIONAL EXPOSURE
Category III exposure to bloodborne pathogens and may encounter chemical hazards.

RESPONSIBILITIES:
1. Admits patients to the Facility following the established policies and procedures.
1.1. Assembles patient medical record forms and prepares patient identification.
1.2.Assists Financial Counselor in obtaining pertinent information to register patient and attaches proper means to document patient identity.
1.3.Provides pertinent information to the patient's family in the waiting area according to policies and procedures.
1.4.Monitors waiting area.
2. Coordinates reception area activities for effective communication with all areas of the Facility.
2.1. Answers telephone and intercom courteously and in a quiet, pleasant voice.
2.2. Accepts and relays messages effectively.
2.3. Informs physician's office of admissions and activities concerning admissions.
2.4. Distributes Surgery Schedule.
2.5. Maintains and protects each patient's right to confidentiality.
2.6. Identifies emergencies and initiates appropriate response.
3. Assumes clerical duties and responsibilities as necessary.
3.1. Assists in ordering and stocking clerical supplies.
3.2. Assists in maintaining cost‑effectiveness by preventing waste of supplies.
3.3. Maintains order and cleanliness of the front desk and lobby area.
3.4. Performs interdepartmental errands when needed.
3.5. Retrieves Facility mail from Post Office Box, if applicable.
3.6. Opens and distributes mail according to Facility policy.
3.7. Prepares all bank deposits following Facility cash controls.
3.8. Coordinates obtaining diagnostic tests and places diagnostic test reports in appropriate section of patient medical record.
3.10. Assists with preparation and maintenance of Facility records and reports.
3.11. Generate and distribute Physician Check List monthly.
3.12 Prepares CQI report from Physician Check List responses.
4. Monitors the nourishment area.
4.1. Stocks nourishment area.
4.2. Maintains cleanliness and order of the area as needed.
5. Monitors computer system for proper daily back-up process.
6.. Coordinates the collection, processing, maintenance, storage, retrieval and distribution of medical records according to established policies and procedures.
6.1. Maintains a filing system that meets Facility requirements for medical records.
6.2. Provides organized storage system for timely retrieval of individual medical records and maintains charge-out and follow‑up controls of records.
6.3. Maintains the confidentiality, security and physical safety of Facility medical records.
6.4. Provides information, according to confidentiality policies, to those parties who are engaged in research or study projects involving patient care and utilization of services.
6.5. Reviews medical records for timely completion, accuracy and for compliance with PRO generic screens; informs the Facility Administrator about delinquent or incomplete medical records.
7. Adheres to established procedures for cross-referencing and indexing medical records.
7.1. Maintains necessary index‑references for Facility needs and following established procedures.
7.2. Maintains the admission register and all other systems involving patient information.
7.3. Maintains the physician/procedure index.
8. Collects statistical data relevant to the operation of the Facility.
8.1. Reviews the contents of medical records to identify information to be extracted.
8.2. Prepares and presents data and reports on approved forms.
8.3. Completes vital statistics on deaths and reportable diseases.
8.4. Upon request, provides information to those involved in research projects and studies.
8.5. Collects data required for support of continuous quality improvement activities.
9. Adheres to medico-legal requirements when answering correspondence and inquiries.
9.1. Maintains and controls the release of information to authorized persons only.
9.2. Prepares records or correspondence according to Facility needs.
