Summary
As an Intake Worker at the Macon County Health Department, you will be a vital part of our team, responsible for assessing client needs and facilitating access to essential health services. This role is crucial in supporting our mission to enhance the well-being of Macon County residents through effective communication and resource management.
Responsibilities
- Maintains liaisons with other home visitors, partner agencies, and other service agencies serving at-risk families, educators, and school staff.
- Coordinates successful completion of recruitment, screenings/assessments, and referrals of eligible children and their families. Manages the transition process for the family.
- Refers 100% of positive screens to the most appropriate home visiting provider within 24 hours and ensures screen is received by provider. If all home visiting providers are at capacity, refer to appropriate community resources and place client on waiting list for home visiting services.
- Collaborates with WIC, Family Case Management, Better Birth Outcomes, High-Risk Infant Follow-Up, and Healthworks Lead Agency.
- Maintains waiting list with a minimum of monthly contacts to monitor availability of services and eligibility of clients.
- Completes face-to-face assessments of home-based families whenever possible.
- Receives input from IDHS Home Visiting model program coordinators and data entry personnel from each site.
- Maintains tracking system for referrals to comply with internal and external reporting requirements.
- Maintains current knowledge of community resources and provides appropriate referrals with regard to physical, emotional, and developmental needs of clients and their families.
- Attends Home Visiting -related events in the community.
- Participates in local early childhood networks, associations, and events serving at-risk children and people with disabilities. Includes facilitation of quarterly Advisory Board meetings.
- Attends professional development classes, training programs, workshops and/or conferences as determined by the IDHS grant and Macon County Health Department (MCHD).
- Exhibits competence with multicultural awareness and resulting effect on nutrition and health practices/problems.
Qualifications
- Minimum of bachelor’s degree in the field of Early Childhood, Social Work, Health or Mental Health, or related field.
