Summary
We are seeking an Assistant to join our team at the Macon County Health Department. This role is essential in supporting our mission by assisting in various health programs and initiatives aimed at improving community well-being.
Responsibilities
- Offers basic, low risk nutrition education and breastfeeding counseling and support.
- Answers to the supervision of the Nutrition Coordinator.
- Provides WIC program explanation to participants.
- Collects and documents participant anthropometric measurements, bloodwork, breastfeeding information and health and nutrition information in WIC MIS.
- Completes WIC secondary education, WIC Certification/Recertification/and Mid-Certification visits.
- Provide referrals to Nutrition Coordinator and or designated CPA for all high-risk, complex nutrition, and breastfeeding problems.
- Provide referrals to other health related and public assistance programs. May include Medicaid, SNAP, TANF, or other food assistance programs (e.g., food banks, food pantries).
- Understands and relays principles of lifecycle nutrition to the WIC participants including selecting a core food package to be issued.
- Completes WIC nutrition assessment process per the WIC and local agency Policy and Procedure Manual.
- Able to maintain WIC Clinic visit pace to meet daily clinic schedule needs.
- Communicates WIC participant visit follow up needs as indicated to WIC/FCM Division team.
- Promotes and supports breastfeeding per contract requirements.
- Develops rapport and fosters open communication with all providers and participants.
- Exhibits competence with multicultural awareness and resulting effect on nutrition and health practices/problems.
- Integrates WIC service delivery for the following programs as needed: WIC BF Peer, Family Case Management, Better Birth Outcomes, Healthworks Lead Agency, and High-Risk Infant Follow-up.
- Maintains current knowledge of community resources related to referral needs.
- Completes documentation and accurate data entry utilizing the WIC MIS data entry system.
- Confers with parents, physicians, dentists, schools, and other groups to gather and receive pertinent information related to assignments with appropriate release of information.
- Participates as needed in Illinois Department of Health and Human Services Program Review procedures.
