JOB OVERVIEW : As the Director of Case Management, you will be the cornerstone of our efforts to deliver high-quality, cost-effective healthcare services. You'll lead initiatives in utilization review, discharge planning, and care coordination across multiple departments, including Emergency, Labor & Delivery, and Behavioral Health.
KEY RESPONSIBILITIES :
- Oversee and optimize case management and social work operations.
- Lead efforts to reduce denials and improve approvals.
- Build strong relationships with physicians, hospitalists, and ED staff.
- Implement and oversee weekly educational programs for staff.
- Drive continuous improvement in length of stay metrics.
- Manage the integration of social workers into the case management team.
- Develop strategies to address challenges in Intermediate Care.
- Collaborate with corporate on utilization reviews.
