Madison Regional Health System is seeking a part-time Social Worker, RN or LPN Case Manager to be a part of the Madison Regional Health System mission: Committed to providing quality healthcare, close to home.
Our Case Manager is an essential part of our Medical team and is responsible for facilitating the plan for an orderly transition of patients from the acute area setting to the next appropriate level and place of care. Responsible to plan, develop, organize, implement, and evaluate all aspects of discharge planning. Implements service s to patients and families; assuring that the medically related emotional and social needs are met/maintained on an individual basis. Maintain contact with referring agencies and related people and with community resources used for referral. Excellent customer service, attention to detail, great telephone etiquette, computer knowledge, and the ability to multi-task in pressure/stressful situations are essential. South Dakota Social Worker, RN or LPN license and CPR certification is required.
The Case Manager part time position is eight hour shifts . The working conditions will be in a clean, well-lighted area, with sitting, standing, walking, bending, stooping, frequent repetitive motion of the hands/wrists involved. May be required to deal with unpleasant environmental situations resulting from patient care or patient activity. Exposure to infectious disease, body fluids, odors, and dust.
