Key Responsibilities
- Conduct resident assessments (MDS) in accordance with all current regulations and guidelines
- Work closely with our interdisciplinary team to establish personalized care plans for residents
- Monitor resident progress and documenting responses to implemented care plans
- Engage in Quality Assurance and Performance Improvement initiatives
