Job Description:

We are seeking a highly skilled and dedicated RN MDS Nurse to join our team at Madonna Manor. The successful candidate will be responsible for conducting thorough resident assessments (MDS) in accordance with all current regulations and guidelines.

Responsibilities:

• Conduct thorough resident assessments (MDS) in accordance with all current regulations and guidelines.
• Ensure the MDS process is completed accurately and promptly.
• Work closely with our interdisciplinary team to establish personalized care plans for residents.
• Monitor resident progress and document responses to the implemented care plans.
• Engage in the Quality Assurance and Performance Improvement (QAPI) initiatives.
• Facilitate effective communication between residents, their families, and other healthcare professionals to ensure superior care delivery.
• Remain knowledgeable about the latest MDS regulations and guidelines for compliance.

Requirements:

• Must hold a current RN (Registered Nurse) license.
• RAC-CT (Resident Assessment Coordinator-Certified) certification is mandatory.
• In-depth knowledge of MDS 3.0, RAI process, and care planning methodologies.
• Exceptional organizational and communication skills, with strong interpersonal abilities.
• Capable of both independent and collaborative work within a team environment.
• Demonstrated commitment to delivering high-quality care to our residents.
