The MDS Coordinator plays a critical role in ensuring that resident assessments are accurate and optimized for individual care plans. Key responsibilities include:
- Conducting thorough resident assessments (MDS) according to current regulations and guidelines
- Ensuring timely and accurate completion of the MDS process
- Collaborating with our interdisciplinary team to develop personalized care plans for residents
- Monitoring resident progress and documenting responses to implemented care plans
- Participating in Quality Assurance and Performance Improvement (QAPI) initiatives
- Fostering effective communication between residents, their families, and healthcare professionals to deliver exceptional care
- Staying informed about the latest MDS regulations and guidelines to ensure compliance

Qualifications
- Current RN (Registered Nurse) license
- RAC-CT (Resident Assessment Coordinator-Certified) certification required
- In-depth knowledge of MDS 3.0, RAI process, and care planning methodologies
- Exceptional organizational and communication skills, with strong interpersonal abilities
- Ability to work independently and collaboratively within a team environment
