**UNIT SUMMARY:
Serving residents in a long term care setting**


**JOB OVERVIEW**

Manages inpatient cases to ensure that patient rights are upheld and that patients are involved in their treatment plans. Advocate for patients in availing themselves of community, social, medical and financial resources. Initiate and implement clinically appropriate discharge plans when appropriate.


**ESSENTIAL FUNCTIONS** * Analyze, compile resident medical, social, and financial information in order to provide a program that meets the resident's needs and preferences. * Compile, coordinate and analyze resident medical and psychosocial data to develop social histories and social work care plans that provide for quality of care and meet individual and resident/family needs, as well as licensure and certification standards. * Mentor, negotiate and instruct long term care residents/families regarding facility services and patient rights. * Ensure resident/family involvement in care planning. * Complete designated sections of the RAI and develop and implement care plans based on results. * Organize, monitor, and lead resident Alzheimer’s support group and Residents' Council. * Instruct clinical team members in necessary psychosocial resident information to enhance care plan development and revisions. * Negotiate with community agencies regarding resident needs and agency ability to meet needs, making appropriate referrals and arrangements to provide for continuity of care. * Speak and negotiate with residents/families and potential funding sources to facilitate patient eligibility for benefits (i.e. Medicaid applications, disability benefits). * Mentor, instruct and negotiate with residents and family members regarding eligibility for and application for specific programs to meet financial, social and/or health care needs. * Instruct and negotiate with residents and family members regarding advanced directives and legal protection in order to enhance patient self-determination and protection. * Speak and negotiate with residents/families regarding concerns/grievances and facilitate resolution to ensure customer satisfaction. * Negotiates and speaks with the Director of Case Management regarding problem case situations to obtain direction and/or support. * Follow instruction from administrative personnel to perform other functions as assigned in order to achieve goals within the facility, division and the clinical team. * Must perceive customer relations as a primary focus, and continually strive to satisfy the needs of every customer in all areas of the work force.

**QUALIFICATIONS** * Bachelors Degree in Social Work from an accredited school of social work. * One year of Social Work experience rquired * Experience with the elderly and/or disabled preferred * Must possess current Nebraska CSW certification or obtain certification within the first three months of employment and maintain current certification.
