TITLE: Master in Social Work

REPORTS TO: Social Work Manager

LOCATION: Hybrid-Mostly remote with rare visits to Ukiah

STATUS: Non-Exempt

JOB SUMMARY:

The Madrone Care Network (MCN) Master in Social Work (MSW) provides direct palliative care social work services as part of the interdisciplinary team. This position reports to the Social Work Manager and supports the clinical team. The MSW actively participates in the delivery of specialty palliative care through education, quality improvement, wellness initiatives, and program development.

ESSENTIAL JOB FUNCTIONS:

· Provides direct medical social work to patients and their families through in-person visits, phone, and video-conference mediums

· Assesses the psycho-social needs of the patient and family and contributes to an interdisciplinary plan of care

· Provides care management, crisis intervention, and short-term psycho-therapeutic interventions to patients and families, as defined by license and training

· Collaborates with the interdisciplinary team to assess and refer patient medical and spiritual care needs to clinical and chaplaincy team members as appropriate

· Assists patients with the completion and understanding of advance care planning documents

· Acts in accordance with MSW training, BBS guidelines, and California mandated reporter legal requirements

· Documents in the EMR, care coordination tracking systems, and additional applications as required for patient care billing and contractual requirements

· Utilizing appropriate data-collection and patient screening tools as assigned (including but not limited to PCQC and PHQ-9)

· Participates in the MCN interdisciplinary team meeting and community multidisciplinary team meetings

· Assists with the solicitation and finding of referrals and the enrollment of new patients as part of the Intake & Assessment Team

· Supports and is able to delegate tasks to SW interns

· Provides palliative care training and education in the onboarding of clinical team members as defined by scope of practice

· Assists with the development of clinical and organizational policies

· Participates in community outreach and community representation of MCN

· Shares expertise with others through in-services and collaboration

· Works with diverse social and economic situations without imposing own values; maintains appropriate professional boundaries and practices cultural competency

· Establishes a team approach and effective, respectful communication

· Contributes to positive atmosphere; demonstrates positive interactions with internal and external customers

· Demonstrates ability to lead and be a productive member of a high performing team

· Provides timely and appropriate response to internal and external customer needs

· Performs other duties consistent with the customary skills and training of an MSW and the mission and goals of MCN as requested

ESSENTIAL EQUIPMENT, TOOLS OR MACHINERY:

Use of the following MCN equipment, with the exception of the employee’s personal vehicle, will be necessary to effectively perform the described job functions:

· Laptop computer or I-pad

· Cell phone

· Personal vehicle.

WORKING CONDITIONS:

Works from home or wherever a patient resides or receives care. Work location must be electronically secure and have stable WiFi and internet connectivity. Requires frequent sitting, walking, and standing. May have some exposure to biological hazards and infectious diseases.

QUALIFICATIONS:

The MCN MSW must:

· Possess a master's degree in social work from a CSWE-accredited program

· Maintain CE requirements for the California Board of Behavioral Sciences

· Have or be eligible for membership in good standing on the medical staffs of hospitals within MCN’s service area

· Be proficient in Word, Excel, PowerPoint and Google calendar/scheduling applications

· Have and maintain a valid Driver’s License, maintain automobile insurance coverage and have access to reliable transportation

· Be able to travel to clinical settings, community agencies and to meet patients in community-based and home-settings as required

· Complete annual TB clearance and training as required

This position description is designed to outline the primary duties and qualifications but not limit the MSW or MCN to the work identified above.

Job Type: Full-time

Pay: $40.00 - $46.00 per hour

Expected hours: 32 – 40 per week
