JOB SUMMARY:
The Madrone Care Network (MCN) Care Coordinator is responsible for outbound telephonic interfacing with patients, caregivers, and partners, and administrative support of care team members.
ESSENTIAL JOB FUNCTIONS:
· Assists with clerical support aspects of care coordination
· Documents patient outreach and interactions in the electronic health record
· Monitors electronic health record inbox
· Requests and obtains medical records
· Orders Durable Medical Equipment
· Supports patients and team members with teleconferencing and visit technology
· Scans files and documents into patient charts
· Maintains the Master Patient List and supports maintenance and auditing of visit frequency requirements
· Schedules home face-to-face visits—either in-person or tele-visits—for team members
· Provides visit reminder calls to patients
· Takes Interdisciplinary Team meeting notes
· Arranges transportation for patients
· Schedules external meetings when requested
· Assists with the completion and signing of patient documents/forms when requested by staff
· Creates staff appointment calendars
· Covers phones when colleagues are on the phone or on a break
· Assists in the overall efficient functioning of the office

The following equipment, with the exception of the employee’s personal vehicle, will be necessary to effectively use in performing the described job functions:

· Photocopier/scanner
· Personal vehicle.
WORKING CONDITIONS:
Work is conducted in an air-conditioned office environment located at 643 S. Main Street in Ukiah. Requires frequent sitting, walking, and standing. Ability to hear and communicate well verbally.
QUALIFICATIONS:
· Current California Driver’s License, auto insurance and vehicle registration
· Related professional work experience
· Advanced computer skills with capacity to access patient records. Able to teleconference with patients and their families
· Capable of hearing and communicating verbally with patients, caregivers, other members of the team, and community partners
· Capacity to work independently with minimum supervision
· Organized with ability to prioritize and multitask
· Ability to respect all patients and caregivers irrespective of their circumstance, ethnicity, race, sexual orientation, religious belief, or political affiliation
Ability to perform the essential functions of the job

Pay: $18.00 - $22.00 per hour
