Qualifications: The Director of Patient Care Services shall fulfill one of the following categories: 1) Registered nurse with a current RN license with a BA or higher degree in nursing or related field and one year of supervisory experience in hospice/home health and three years of additional clinical experience. 2) Registered Nurse with a current RN license, with three years supervisory experience in hospice/home health and three years of additional clinical experience.
Functions: - Assess patients referred to determine appropriateness of hospice care and coordinate admission process - Assign patient/family to appropriate staff - Facilitate the interdisciplinary team meetings around the patient/family plan of care - Implement and monitor compliance with agency policies including Safety, Infection Control and Patient Care - Coordinate with ED to facilitate staff meetings for information, support and training - Visit patient/family as needed for the purpose of continued assessment as well as supervision and support of staff - Supervise maintenance of medical records, assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards - Promote efficient and effective utilization of resources consistent with agency procedures - Identify need for staff education, coordinate in-service programs and keep training records - Participate in staff selection and orientation of new personnel - Coordinate physician/agency relationships, community education and information - Communicate regularly with Executive Director concerning overall quality of hospice care. - Report on issues that concern risk to patients or family and liability of Madrone Hospice.

Pay: $44.06 - $54.86 per hour
Expected hours: 32 – 40 per week
