POSITION SUMMARY
We are seeking a highly motivated and dependable individual for a full-time salaried position. The QDDP/HS is responsible for developing Individual Support Plans and goals at one of Magdala’s group homes (9 individuals total). The QDDP/HS also supervises and trains direct care staff, oversees staff schedules to ensure adequate coverage is provided, and is instrumental in managing the daily functions of the home.
A successful candidate is a person who is responsible and reliable. They must have exceptional leadership and communication skills, experience in supervision, and, excellent writing skills, strong organizational skills.
FUNCTIONS OF THE JOB
- Leading and directing the service delivery team in supporting 9 individuals with developmental disabilities in a manner that provides self-determination and quality life opportunities to the persons being supported
- Writing and implementing Individual Support Plans
- Writing and implementing treatment goals
- Training staff on Individual Support Plans and goals
- Ensuring data and program documentation is completed by staff daily
- Managing the daily operations of two group homes
- Hiring and training Direct Support Staff
- Providing all necessary performance discipline
- Ensuring quality staff are available for house coverage, doctor’s appointments and community activities
- Communicating relevant information to families, DMH Service -Coordinators, and Magdala team members
- Ability to be on-call 24 hours day M-F and to participate in a weekend rotational call schedule
- Overseeing the budgeting and documentation of funds spent for the individuals being served
- Understanding of Positive Behavior Supports and providing on-going feedback to staff regarding correct implementation of Behavior Support Plans
- Keep supervisor informed of events, concerns, and needs, as appropriate
- Ensure that client files are maintained according to licensure and certification standards
SKILLS AND ABILITIES
- Experience with multi-tasking and the ability to prioritize and organize work duties in a manner that ensures deadlines are met.
- Knowledge of Department of Mental Health procedures/standards
- Excellent supervisory skills and ability to provide leadership to staff
- Ability to communicate with a diverse group of people
- Ability to make sound decisions based on analysis, experience, and good judgment
- Willingness to “wear many hats” and do what it takes to get the job done
- Ability to anticipate, identify, and resolve problems in a timely manner
- Consistent modeling of appropriate and positive interactions for staff and clients

MINIMUM QUALIFICATIONS
- Bachelor’s Degree in the Human Services field
- A minimum of one year working directly in the field of Developmental Disabilities. Three years of experience preferred.
- Two years supervisory experience preferred.
- Ability to respond quickly to emergency situations.
- Flexible availability and ability to be on-call.
- Valid driver’s license with a clean driving record (no major violations in the past 5 years).
- No more than 1 moving violation or 1 “at fault” accident within the past 3 years.
- Must be at least 25 years of age (depending on insurance requirements).
- Ability to pass a motor vehicle background check and maintain eligibility to drive company vehicles.
- Comfortable driving residents in both company vehicles and, if required, personal vehicles with appropriate insurance.
- Clean criminal and EDL record
- Nonsmoker, including vaping, preferred
