Maggie's Hospice and Palliative Care is looking for an extraordinary social worker with a MSW degree (Masters of Social Work) with a hospice heart who wants to work for a company that cares. We have APRN positions available, with the potential to be full time (if desired) as we grow. We have an amazing team and are looking for quality people as we continue to grow. We are dedicated to providing compassionate hospice care to those in need. Hospice experience is preferred.
Must be willing to travel to wherever the patient calls home!
This is a PRN position for Flagstaff and surrounding areas. PRN staff actually get paid per visit, not by hour, so the rate listed is just approximate.

Duties:
- Conduct assessments and evaluations of individuals to determine their needs and develop appropriate treatment plans
- Collaborate with other professionals, such as Nurses, physicians, Chaplains and educators, to develop comprehensive treatment plans
- Provide counseling and support to individuals and families during end of life situations.
-Assist individuals in accessing community resources and support services
- Maintain accurate and up-to-date documentation of client interactions and progress
