Magnet Medical is seeking an experienced nurse to join our team as a Progressive Healthcare Coordinator in the Progressive Care Unit (PCU). As a key member of our interdisciplinary team, you will be responsible for coordinating patient care and contributing to the development of individualized care plans.
Responsibilities:
- Patient Coordination: Coordinate patient care and facilitate communication between healthcare professionals to ensure seamless transitions and high-quality care.
- Patient Education: Educate patients and families about the recovery process, treatment plans, medications, and any required lifestyle changes.
- Documentation: Accurately document all nursing interventions, patient assessments, outcomes, and any changes in condition using electronic health records (EHR).
