OVERALL JOB PURPOSE
The overall purpose of the Wellness Nurse position is to provide Person-Centered Care consistent with accepted standards and expectations of Sunshine Retirement Living and state regulations. The Wellness Nurse responsibilities include, but are not limited to, monitoring resident health status, providing physical care, and early diagnosis and intervention of resident changes in health status. Wellness Nurses must communicate politely and effectively with family, primary physicians, and perform all administrative charting or other services as required by regulations and company policy.
MINIMUM JOB QUALIFICATIONS AND REQUIREMENTS
- A current, valid LPN or RN license is required
- Education / and or experience in long-term care, acute care, or geriatric nursing preferred
- Supervisory experience preferred
- Ability to read, write, and speak the English language and to verbally communicate with different levels of staff, families, and any outside customers in a professional and polite manner
- No disqualifying criminal or care offenses as defined by regulatory guidelines
- Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to work under time constraints and meet deadlines
- Maintain a positive and respectful approach with coworkers. Ability to work in a team setting and be a team player
- Ability to write nursing / patient progress notes, and nursing procedures
- Ability to effectively present information and respond to questions from department heads and customers (patients, family members, physicians, etc.) in a polite and professional manner
- Satisfactorily pass Sunshine’s and state regulated criminal record clearance, health screening and TB clearance (if applicable)

Values and upholds the Sunshine Retirement Living Principles and Values including compassion, responsiveness, teamwork, courtesy, and personalized careDedicated to delivering a high level of customer serviceProvides resident care in accordance with accepted standards of practice and within the scope of the LPN or RN licenseRoutinely observes and reports on resident’s conditions / changes and then documents in accordance with community policies and as required by regulationsAdministers medication / treatment as prescribed within the LPN or RN scope of practiceReceives, transcribes, and executes physician ordersImplements and evaluates resident’s plan of careIdentifies and secures equipment and supplies. Notifies supervisor when supplies are neededEnsures appropriate and timely response to resident and / or family concernsResponsible for the safety of residents under your supervisionInforms appropriate staff about the condition of residents and expectations / needs for the shift assigned at the beginning of each shift and receives report from coworkers throughout and at the end of each shift regarding resident conditionsDirects Caregivers to assure care provided per standards of practice and per community policies and regulationsRounds with Caregiver prior to end of shift to assure unit is in proper orderPerforms other duties as assignedOTHER JOB DUTIES AND RESPONSIBILITIES
Adhere to the Resident Rights and promotes safety, dignity, and respect among all residentsEnsure safety of residents through appropriate behavioral interventionsPromotes employee relations, facilitates communication and problem solvingEnsures that community staff follow facility policies and proceduresCoaches, mentors, and counsels resident care staff as needed and keeps Health Services Director informed of any concernsEnsures appropriate and timely response to customer service concernsDemonstrates knowledge of resident care and its impact on residents and familiesAssists resident care staff in performing duties as neededAssists with immediate incident reporting and analysis of incidents and corrective action, when appropriateMaintains confidentiality of community and resident informationAttend community meetings and in-service trainings as requiredKnow the emergency plan, fire evacuation routes, extinguishers, and other facility safety featuresCart and chart audits as scheduledClinical Evaluation SkillsCreate monthly resident lists for Home Health, Hospice, and OxygenAbility to work with 911 and attend to emergency callsAbility to work / run a medication cartReport to work on time as scheduled and consistently demonstrate dependability and punctuality. Consistent and regular attendance is requiredAccept assigned duties in a cooperative mannerComplete all other duties assigned by the Health Services Director and / or Executive DirectorMaintains certifications as necessary such as First Aid, to fulfill state regulations (if applicable)This position has supervisory responsibilities of the care staffPerform all work assigned for the agreed-upon salary, accepting no additional payment or non-perishable gifts from residents, vendors, Community Management, or othersMeet the assigned work requirements of the job. Monday and Tuesday 12 hours : 7pm to 7am
Every other Saturday and Sunday 8 hours : 11pm to 7am
