JOB OPENING
POSITION : Service Coordinator, Early Intervention
DUTIES (include but not limited to the following) :
- Establish and maintain positive relationships with assigned families through regular home visits and monitoring, serving as single point of
contact to assist families throughout their Early Intervention (EI) journey beginning with initial contact through transition at age 3
Actively participate as a member of Mahoning County's Evidenced-Based EI TeamShare and ensure parent's rights within EI system and completerequired consents
Observe developmental skills of infants and toddlers; conductscreenings as appropriate
Gather medical, developmental and social history information to present to teamCoordinate eligibility and enrollment process including coordination of developmental evaluation and assessmentConduct family-directed assessment to identify family's prioritiesProvide information about community resources and make relevantreferrals
Coordinate and facilitate the development and review of theIndividualized Family Service Plan (IFSP) that identifies needed EI
services and their funding sources
QUALIFICATIONS / REQUIREMENTS
include but not limited to the following) :
Bachelor's Degree in Social Work, Early Childhood Education orrelated field
Eligible for Service Coordinator credential issued by the OhioDepartment of Developmental Disabilities (DoDD)
Possess knowledge of available community resourcesAbility to interact in a sensitive and supportive manner with families and guardiansAbility to efficiently plan to meet tight deadlinesAbility to negotiate and manage conflictAbility to use coaching strategies to support parentsAbility to work variable and flexible hours, including eveningsAbility to attend required in-service trainings as offered by DoDDEvidence of a valid driver's license in state of residence, good driving record and full liability coverage on any vehicle used for transportationExcellent interviewing and documentation skillsGood organizational and time-management skillsExceptional communication skills (verbal, written, and interpersonal) for effective interaction with internal and external customersSelf-starter with ability to work independentlyPossess computer skills in various word processing and database management programsDemonstrated commitment to valuing diversity & contributing to aninclusive working environment
