POSITION TITLE:              Assessment Coordinator

PROGRAM:                      Older Alaskans Program

REPORTS TO:                   Program Manager

FLSA STATUS:                  Full-Time / 40 hours per week; Non-exempt

Scope of Position: This position is responsible for the initial introduction to services for Medicaid Waiver clients, for any follow-up of authorized clients, and coordination with care coordination system. The position also promotes the Older Alaskans Program (OAP) services in the community and to individual clients.

Minimum Qualifications:

  1. Associates Degree in Human Services or equivalent related work experience.
  2. Ability to accurately report functional, physical, social, and mental status of individuals and determine suitability for supportive services and care coordination.
  3. Demonstrated skill and sensitivity in communicating with seniors, the disabled, and those who use English as a second language.
  4. Proficient in using a computer and applications including Excel and Word.
  5. Able to use basic office equipment. (ex: copiers, fax, printers)
  6. Ability to maintain excellent working relationships and work collaboratively and effectively with others.
  7. Work independently with minimum supervision when required.
  8. Current, valid Alaska driver's license.

Essential Functions:

  1. Provides information on OAP services.
  2. Handles initial telephone intake for potential clients. When appropriate, refers clients to other providers for services unavailable through OAP.
  3. Completes the assessment tool on potential and authorized clients to identify their needs and determine eligibility for services
  4. Coordinates with care coordinators, family members, and other caregivers to determine needs for clients.
  5. Provides client follow-up as needed based on delivery services, complaints, or other issues, and may arrange for home visits; confers with supervisor regarding unusual client issues.
  6. Maintains contact with care coordinators to assure completions of prior authorizations and other paperwork.
  7. Completes annual re-assessment to evaluate client needs for continued services.
  8. Maintains accurate client case notes and files; monitors waiting list and contacts clients to determine current situation.
  9. Assists in planning and carrying out targeted outreach which may include attending and participating in staff meetings, workshops, programs, public displays, and health fairs.
  10. Promptly and accurately reports any incidents of suspected or known elder abuse.
  11. Assists with completion of statistical data or reporting as needed.
  12. Performs other related duties as assigned.

    Physical Demands: Speaking, listening, sitting, standing, bending, stooping, kneeling, pushing, pulling, climbing stairs and balancing, and walking are required. Manual dexterity necessary to operate keypads and keyboards. Visual and hearing acuity at or corrected to normal range is required to communicate by telephone and in person with others. Must be able to lift to 20 pounds. Ability to operate a motor vehicle in daylight, darkness, and in inclement weather conditions.

    Qualified individuals must be able to perform the essential duties of the position with or without accommodation. The Salvation Army will attempt to satisfy requests as long as the accommodation needed is reasonable and no undue hardship will result.

    Environment: Work is performed in a well-lit, heated, and ventilated office environment. Noise level is generally low. The use of personal protective equipment may be necessary during periods of exposure to clients who are ill during home visits. Exposure to extreme weather conditions is not a regular requirement of this position.

    Language Skills:  Must be able to understand, follow and give both written and oral instructions. The employee must be able to use speech and hearing for ordinary and telephone conversation.

    Mathematical Skills:  Must be able to perform simple calculations (addition, subtraction, multiplication, and division) and apply concepts such as ratios and percentages in practical situations.

    Performance Skills:  Must be creative, self-motivated, organized, flexible, and possess leadership skills.

    Sacred Boundaries and Protect the Mission:  Must complete the online course for this area and pass the Background Check required by The Salvation Army for employment.
