Enhanced Care Management (ECM) Specialist

BASIC PURPOSE

The purpose of the ECM Specialist is to provide comprehensive care coordination for individuals experiencing complex medical and/or behavioral health needs.

1. ESSENTIAL DUTIES AND RESPONSIBILITIES
2. Conduct intake and program eligibility for Enhanced Care Management services
3. Manage assigned cases actively receiving Enhanced Care Management services
4. Conduct case management and provide service coordination
5. Transport participants for service navigation and medical appointments
6. Advocate for assigned participants to respective outside agencies
7. Actively participate in all collaborative partnerships as assigned by supervisor
8. Coordinate medical referrals in coordination with contracted managed care plans
9. Participate in weekly case conference meetings
10. Participate in program and agency trainings as assigned
11. Maintain accurate participant records in various information management systems; and generate reports as requested
12. Maintain and execute confidential information according to HIPPA standards
13. Maintain a highly detailed and organized filling system
14. Ensure intake procedures utilize trauma informed care, harm reduction and housing first principles
15. Check and respond to emails and voicemails on a regular basis
16. Adhere to confidentiality standards
17. Other duties as assigned

KNOWLEDGE, SKILLS, ABILITIES AND OTHER QUALIFICATIONS REQUIREMENTS

1. Ability to speak and write the English language at a high and professional level
2. High degree of confidentiality
3. Able to endorse and promote The Salvation Army’s mission
4. Minimum two (2) years of social service/work experience preferred
5. Computer literate, in Windows environment (Microsoft Office), Publisher, Access and Outlook preferred
6. Excellent communication skills, both written and verbal.
7. Excellent and professional telephone etiquette and presence
8. Excellent organizational skills
9. Strong ability to utilize a high level of time management and handling multiple tasks

CERTIFICATES, LICENSES, REGISTRATIONS

1. Associate's degree in Social Work, Business Administration, Human Services, Management or minimum of 2 year's experience in social work, homeless service management, human resource management, healthcare management/administration or related field
2. Must possess a valid California Class C Driver License, and ability to drive a Salvation Army vehicle
3. Must be 21 years or older
4. Authorize The Salvation Army to add name to the CA DMV Pull Notice Program
5. Complete The Salvation Army vehicle course training

PHYSICAL REQUIREMENTS:

1. Ability to sit, walk, stand, bend, squat, climb, kneel, and twist on an intermittent or sometimes continuous basis
2. Ability to grasp, push, and/or pull objects
3. Ability to reach overhead
4. Ability to operate telephone
