What You Will Do to Change Lives
The Clinician provides mental health support services to members served and their families at the program. The Clinician also assists in the coordination of all mental health support services provided by the Team. This includes coordinating and collaborating with team members and outside agencies such as, the Conservator’s Office, Public Guardian’s Office, Probation Department, County Behavioral Health Services, and housing and inpatient providers.
Shift Available:
Full Time Salaried; 8:00 am - 4:30 pm; Monday - Friday
Expected starting wage range is $68,033.91 - $84,024.91. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
What You Bring to the Table (Must Have)
- A Master’s Degree in Social Work or related field.
- Registered with the CA BBS with a ASW, AMFT, APCC
- Two (2) years of experience post masters in mental health setting
- Personal car that you are willing to use for work
- Willingness to transport clients in your personal car
- Driver's License that is valid and in good standing

To Make Referrals:
- ICM: For institutional case management, referrals come directly from the Public Conservator’s office and/or coordinated through San Diego’s Department of Behavioral Health. This program only serves individuals that are on public conservatorship.
- SBCM: For strength-based case management, referrals come from a variety of places including hospitals, other providers, family members, and individuals. All referrals must be submitted through Optum’s Single Point of Access (SPOA) by submitting an online referral form Single Point of Access-ACT & Strength Based Case Management (optumsandiego.com)
# of Members: 354 ICM members; 352 SBCM members
Who We Serve: Adult residents of San Diego County, ages 60 and older, who are diagnosed with a serious mental illness. For ICM services, the individual must also be conserved and placed in a locked, residential facility.
