What You Will Do to Change Lives
The Community Health Worker assists in providing outreach and engagement with eligible Managed Care Plan members and supporting care management through transportation assistance to assigned appointments and social services.
Shifts Available:
Full Time; AM 8:30 am - 5:00 pm; Monday - Friday
Expected starting wage range is $21.00 - $23.24. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
ESSENTIAL FUNCTIONS
· Demonstrate the Telecare mission, purpose, values and beliefs in everyday language and contact with the internal and external stakeholders
· Provides daily support to Lead Care Coordinator related to care coordination for members
· Engages and conducts in-person outreach with eligible Managed Care Plan (MCP) Members
· Accompanies MCP Member to office visits, as needed, and in the most easily accessible setting, within MCP guidelines
· Distributes County provided health promotion materials
· Extends County provided health promotion and self-management training
· Arranges or provides transportation when needed and appropriate
· Provides assistance with linking member to social supports
· Contacts MCP Member to facilitate visit with care coordinator
· Advocates on behalf of MCP Members with health care professionals
· Uses motivational interviewing, trauma-informed care, and harm-reduction practices
· Monitors treatment adherence (including medication)
· If employed at Inpatient program, must assist with restraint of members served in the event of assaultive behavior and pass assault crisis/crisis prevention training
· If employed at Outpatient program, must evade members served in the event of assaultive behavior and pass assault crisis/crisis prevention training

What You Bring to the Table (Must Have) Required:
· High School Diploma or GED required
· Two (2) years’ experience providing mental health or intensive case management services and coordination
· Paraprofessional with or without lived mental health experience
· Experience supporting individuals with physical health challenges to navigate the physical health care system
· Must be at least 18 years of age
· Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment
· All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, valid driver license, a motor vehicle clearance and proof of auto insurance is required at time of employment and must be maintained throughout employment. Additional regulatory, contractual or local requirements may apply.
- Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc. (weekly mileage reimbursement at the IRS rate)

SKILLS
· Knowledge of recovery focused, strengths based work and whole person care
· Ability to work effectively as a member of a multidisciplinary team
· Thorough understanding of the rights of members served
· Understanding of managed care systems
· Knowledge of basic medical and psychiatric terminology
· Strong knowledge of relevant community resources and methods for accessing them
· Possess strong communication, writing skills, and typing/computer skills
· Ability to communicate and work effectively with members served, family members, and staff from various backgrounds
PHYSICAL DEMANDS
The physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
The employee is occasionally required to stand, twist, squat, kneel and lift and carry items weighing 25 pounds or less as well as to frequently sit, walk, reach, bend and do simple and power grasping. The position requires manual deviation, repetition and dexterity and to occasionally drive and be exposed to uneven walking ground as well as Hazardous Exposure (Blood Borne Pathogens, Hospital Waste, Chemical & Infectious). Visual requirements include computers and books exposure.
