Roles & Responsibilities
The Case Manager (also known as a Community Integration Worker) is responsible for the identification, assessment, planning, linking, monitoring, and evaluation of services and support needed by the member with a mental health diagnosis that interferes with their ability to reach stability.
Case Managers work with the member to identify and make progress on goals that assist the member with managing mental health symptoms and connecting them to services and support to assist them with living as independently as possible within their community. These areas include their emotional, psychological, social, financial, dental, medical, recreational, educational, vocational, and housing needs.
Necessary Skills
- Must possess strong time management and organizational skills.
- Ability to multitask, prioritize, and problem solve in a fast-paced environment.
- Demonstrate the ability to produce satisfactory work under pressure and meet deadlines.
- Excellent verbal and written communication.
- Reliable transportation and willingness to transport clients in personal vehicle.
- Formulate 90-day individual treatment plans with member that include measurable goals.
- Document case notes daily and submit weekly.
- Have basic computer knowledge.
Pay
- Up to $25/hr with potential for differential pay rates if desired.
Minimum Requirements
- Active MHRT-C, or MHRT-C provisional.
- Valid Maine Driver’s License.
- Active motor vehicle insurance & registration.
Preferable Requirements
- 1 year experience in the mental health field.
- Trained in KEPRO.
- Familiar with electronic documentation.
- Familiar with components of a qualifying clinical letter.
- Understanding and use of MaineCare terminology.
