Description
The Residential Treatment Nurse provides direct nursing care and health support to individuals in the residential program. Working under the supervision of a Registered Nurse (RN), the Residential Treatment Nurse assists the healthcare team to assess individuals’ needs, plan and modify care for the people we support. This position plays a key role in monitoring and managing the health needs of residents while supporting a safe, therapeutic environment.
Responsibilities:
- Work alongside Coordinators and Mangers to ensure follow through and resolution of all health issues. Ensure that physicians’ orders are followed, and outcomes reported to the necessary care providers.
- Work as a member of cross-departmental project teams with the purpose of continually improving quality of care.
- Serve as a team member for the organization’s Quality Improvement Plan.
- Participate in discharge planning as needed. Oversee the coordination of outside medical services such as PT/OT/ Speech therapies.
- Oversee the delivery of specialized medical equipment and assist staff with utilization as needed.
- Ensures that all programs are adequately supported for all medical needs and requirements as per 6400 Regulations.
- Review StationMD telehealth appointments to determine the appropriateness of the appointment, work with coordinators and managers to identify any follow-up care that is needed, and work with the Quality Department to identify trends and key takeaways.
- Assist Managers with coordination of medical care for individuals with all medical providers (including PCP, specialists, and dental)
- Review and ensure that appointments are entered in Therap appropriately
- Assist Managers with training staff on Therap /electronic medical record
- Ensure that annual appointments are completed annually to ensure HEDIS Gaps are closed
- Participate in and prepare for Quarterly case consults with UPMC
- Work as part of a healthcare team to assess individuals’ needs, plan and modify care.
- Work with program operations staff to prioritize and implement HRST Considerations, ensuring that staff have appropriate training and support.
- Provide Nursing Oversight for individuals with complex medical conditions and be a resource for families and care givers.

- Physical Requirements for the Position: ability to lift 50 pounds regularly, ability to respond quickly to sounds, ability to move safely over uneven terrain or in confined spaces, ability to see and respond to dangerous situations, ability to work in extreme weather, ability to wear personal protective gear correctly most of the day
Education and Experience
- Requires an active PA Licensed Practical Nurse.
- Pediatric experience preferred
- Valid Pennsylvania driver's license
- Use of personal vehicle may be required.
