Summary Of Position

Mainstream Living provides services designed to enhance opportunities, create success and fulfill dreams. The Team Leader position is a professional direct staff position which provides oversight to a designated caseload of members residing in a variety of residential settings; and provides direct supervision to Direct Support Professional (DSP) staff and Assistant Team Leaders (ATL). This position is responsible for the overall direction, coordination, and service implementation to the members assigned to their caseload. This position is also responsible for the training and ongoing supervision of the personnel providing direct care to the members. This position may also be assigned to provide or arrange specific skills or supports, conduct activities including appointments and community activities and implement management functions, including policies and procedures relating to IAC 78.41(1).

Qualifications And Skills

Education: A Bachelor's degree in Human Services or related field is preferred.

Must have High School Diploma or Equivalency/HiSet/GED. *

Experience: 2 years Human Service Experience or supervisory experience is preferred.*

  • Combination of education and experience will be assessed for determination of qualified applicants.

Driving: This position requires possession of valid Driver's License, legal driving privileges and ability in the State of Iowa and qualification with Mainstream Living's underwriter as an insurable driver.

Computer: Basic data entry skills required. Familiarity with Word Processing, Google Docs, Microsoft Excel/spreadsheets is strongly preferred.

Post hire: Must demonstrate competencies to complete internal training, including training to pass medication.

The Team Leader position requires good attention to detail and superior organizational skills. Excellent communication and interpersonal skills are required, with ability to listen effectively, respond appropriately, and maintain mutual comfort level while relating to diversity. Must have the ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Have the ability to interpret a variety of instructions, furnished in written, oral, diagram, or schedule form.

The Team Leader is a leadership position. This individual must be a stable, mature, professional person capable of working in stressful situations. Must possess good judgment in crisis situations and demonstrate the same. The Team Leader must be flexible, and able to collaborate with the team to develop strategies and systems that resolve problems. Must be able to objectively examine situations and determine appropriate courses of action.

The Team Leader must both serve as a role model for members and DSP staff; and act as a role model for the company by maintaining positive and professional relationships with members, guardians, staff, case managers and other stakeholders regarding direct service and specific needs of the member.

Essential Duties And Responsibilities

This position is considered an essential health care worker position and in the event of an emergency may be required to work one-on-one with members to ensure their safety.

Supervisory Responsibilities

  • Supervises Assistant Team Leaders (ATL) and Direct Support Professionals (DSP).
  • Carry out supervisory responsibilities in accordance with the organization's policies and applicable law.
  • Play an integral role in recruitment, selection, onboarding/transition, and retention of direct reports (ATL and DSP).
  • Support and develop staff to provide optimal services to members, including the monitoring of training to ensure completion and comprehension.
  • Maintain methods for and oversee shift to shift communication. Complete daily review of Communication Book and address challenges and opportunities. Plan and conduct routine staff meetings and 1:1's.
  • Provide timely and consistent feedback and discipline to staff. Assess risk and impact of performance and/or conduct to facilitate initial inquiry. Consult with supervisor, and Human Resources as appropriate.
  • Ensure required documentation is submitted by DSP on time e.g., incident reports, medical information, expense reimbursement, receipts, etc. Maintain communication with internal stakeholders responsible for review.
  • Develop staff schedule templates that meet both member supervision and budgetary needs, with desirable/hireable shift packages to degree possible.
  • Assist ATL with scheduling requirements; monitor overtime and spot check accurate reporting of staff work times.
  • Participate in on-call rotation. Work direct care shifts when necessary.

Member Related Duties

  • Ensure that Mainstream Living policies and procedures and regulatory standards and guidelines are implemented appropriately for each member within their living environment.
  • Participate in the selection of new members and conduct member intake tasks. Establish regular and consistent contact during the transition.
  • Monitor member sites and living environments. Implement systems to address safety concerns, trends or hazards.
  • Be an expert in the the development and monitoring of individualized member program goals, objectives, supports to ensure members are safe, happy, and empowered. Monitor rights restrictions on a routine basis to provide the least restrictive environment necessary.
  • Check medication areas and follow up on any medication errors, organizational issues, outdated medication, drug destruction, etc.
  • Complete serious/critical incident reports and related followup; oversee member crisis intervention and risk management planning. Assist in developing and implementing corrective action plans.
  • Assist in investigations and grievance resolution procedures as necessary to meet the needs of members and their DSP staff.
  • Be physically present for matters of importance e.g., meetings with guardians or other stakeholders, specialty medical appointments, member team meetings, annual team meetings or assessments.
  • Attend and participate in the member assessments. Coordinate with member Case Managers, parents, guardians, and others as needed, to facilitate individualized service provision.
  • Promote members' health and well-being in a manner which is satisfactory to the member and their team, and Mainstream Living values, ensuring that routine and non routine appointments are scheduled, attended; staff are available and ongoing follow up appointments occur.
  • Communicate member medical and/or health needs with nurse/medical staff as necessary and in a timely manner, including not limited to member illness, injury, medication, hospitalization, or serious incident. Ensure immediate steps are taken as needed and follow up steps or checks are completed. Monitor member hospitalizations.
  • Assist in the development, training, implementation and monitoring of member behavioral support plans.
  • Maintain routine and timely communication that fosters positive and professional relationships with members, guardians, staff, case managers and other stakeholders regarding direct service and specific needs of members, including quarterly updates with MCO's.
  • Complete protocols for member discharges or transfers.

General administrative

  • Attendance and communication are essential to this position. Although there is flexibility in setting personal work schedules, it is essential to be physically present for meetings or events, site or member visits, appointments,etc. Remote attendance may be proactively approved by Program Coordinator or designee, on a case by case basis but shall not be the standard.
  • Maintain routine communication that fosters positive and professional relationships with other MSL departments, including timely follow up to email/phone inquiries and other requests.
  • In consultation with ATL, communicate with HR or Payroll personnel regarding employee matters, e.g., employee leaves/FMLA, status changes, transfers, work comp injuries, terminations, employee conduct/performance, etc. Ensure necessary reports or follow up are completed in a timely manner.
  • Follow HIPAA and agency confidentiality guidelines for all communication or agency documentation that occurs or is taken off work premises.
  • Monitor individual or member site budgets and ensure time worked reflects efficient services from staff and that overall service provision meets the requirements of each individual's plan.
  • Monitor and account for agency funds accurately e.g., petty cash, purchases.
  • Complete special projects as assigned and attend internal and external committees/meetings as assigned, acting as professional representative of the agency.
  • Participate in ongoing personal development and keep informed of news/changes from internal and external parties.

Adherence to Policies and Procedures

All employees will adhere to Mainstream Living's policies and procedures including but not limited to:

  • Personnel Policies and Policy manuals.
  • Adhere to Mainstream Living's Attendance Policies for time off, on-call and attendance to training/staff meetings.
  • Follow medication and medical policy and procedures to include: administration, record keeping, identification of medication, purpose, therapeutic intent and adverse reactions, and self-administration of medication.
  • Follow documentation procedures to include accurate, honest and timely completion.
  • Follow health and safety practices per Mainstream Living's policies including participation in emergency procedures, drills, proper infection control, accident/injury prevention, etc.
  • Follow vehicle policies for safe operation of Mainstream Living's vehicles and safe transport of members/others if in driving position.
  • Keep supervisor informed of critical and non-routine events. Communication is constructive, effective, and timely.

WORK ENVIRONMENT

Physical Demands: While performing this job, employees must be able to meet the physical demands required to complete essential functions of this position and meet member needs in their home environment. Employees will be required to lift 15-25 pounds on a frequent basis. Depending on member needs and in some work locations, employees may be required to assist with ambulation, lifting and transferring of members, generally with the use of adaptive equipment such as gait belts, walkers, wheelchairs, mechanical lifts, etc. Thus, there may be times the employee must lift up to 50lbs and rarely up to 100lbs if working in sites with members requiring ambulation assistance.

OSHA CLASSIFICATION

This position is considered a Classification I in regard to jobs in which required tasks routinely involve a potential for mucous membrane or skin contact with blood, body fluids, tissues or potential spills or splashes. Additionally, there is possible exposure to cleaning chemicals. Use of appropriate measures is required for every health care provider.
