What are the responsibilities and job description for the LPC/LMHP/LCSW/LMFT Counselor position at Mainstream Mental Health Services, Inc.?
*$2500 Sign-on Bonus* We are a well-established community behavioral health agency with a strong reputation for providing high quality services. We offer competitive pay and benefits. We are currently looking to add a Licensed therapist to include but not limited to LMHP, LPC, LMFT, LCSW.
- Performs assessments and reassessments of individuals qualifying for outpatient services.
- Assures that documentation is staying up to date on coming due quarterlies, ISPs, and assessments; to ensure no lapse in service and adhere to DMAS standards and best ethical practices.
- Coordinates and carries out and documents the consumer discharge process, including identification of resources for transition to other appropriate and necessary supports.
- Attend to referrals in a timely fashion.
- Assures all required consumer information and documentation is entered in the clinical record in a timely and accurate manner. This will include periodic clinical record reviews.

- Knowledge, skills and abilities in areas related to mental health, mental deficiency and substance abuse, behavior management and community resources.
- Thorough training and/or experience in writing and implementing consumer assessments and service plans and dealing with difficult or problematic behaviors.
- Must have good written and verbal skills, time management and organizational skills.
- Able to establish professional relationships with staff, consumers, and other professionals.
- Must have knowledge of Human Rights, Licensing and Medicaid Regulations.
