JOB SUMMARY AND SPECIFICATIONS
JOB SUMMARY
o Under general supervision, performs community outreach, facilitates individual and group health education sessions, provides client referral to care and services, and builds partnerships with local community organizations to promote health, reduce disparities and improve service delivery. Links resources of MHP with the understanding of the socio-economic needs and culture of the populations / communities served to identify barriers to individual health and to advocate for changes to improve community health. Upholds the mission, vision, values, and customer service standards of MHP.
MINIMUM QUALIFICATIONS
Professional & Technical Skills
One-year experience working with clients in a community-based setting. Highly skilled in the use of personal computers, including, but not limited to, Microsoft Office applications. Experience as a Peer Support Specialist or simiar role. Ability to be on-call for emergencies and work on weekends and public holidays. Community Health Worker (CHW) certification from an accredited program.
Education
High school diploma equivalent.
License(s) or Certification(s)
Peer Support Specialist credential.
Valid driver's license and proof of insurance to be validated at least every six months.

Ability to handle self well in stressful situations; ability to communicate well with others; ability to work individually as well as part of a team; ability to work and communicate effectively with diverse populations; ability to demonstrate flexibility, initiative, and professionalism; ability to multi-task . Life experiences reflective of the community / target population served. Outstanding interpersonal, administrative, and organizational skills. Must be able to speak both Spanish and English with a high level of fluency.
